亚低温改善急性大面积脑梗死患者的神经功能及对血清超氧化物歧化酶、丙二醛的影响

摘　　要：目的:观察亚低温技术治疗急性大面积脑梗死的临床疗效和对血清超氧化物歧化酶(SOD)、丙二醛的影响,探讨亚低温的脑保护作用机制。方法:随机将46例急性大面积脑梗死患者分为治疗组和对照组各23例,对照组常规治疗,治疗组在药物治疗脑梗死的同时加用亚低温治疗,并在治疗前及治疗后14d进行神经功能缺损评分及血清SOD和丙二醛的测定。结果:治疗组较对照组神经功能缺损评分明显降低犤治疗组(13.67±6.24)分;对照组(18.64±8.67)分,P<0.05犦,血清SOD活性显著提高犤治疗组(1.91±0.26)nkat/L;对照组(1.56±0.24)nkat/L,P<0.01犦,丙二醛水平明显降低犤治疗组(9.54±1.42)μmol/L;对照组(11.46±1.63)μmol/L,P<0.01犦。结论:亚低温治疗技术可以明显改善重症脑梗死患者的神经功能,与其清除自由基有一定关系。AIM:To observe the clinical effects and changes of mild hypothermia therapy(M HT) on the serum activity of superoxide dismutase(SOD),content of malondialdehyd e(MDA) in patients with acute cerebral infarction of big area, and to explore th e protective effect of MHT to brain. METHODS:A total of 46 patients with acute cerebral infarction of big area were randomly divided into treatment group(n=23) and control group(n=23).Drugs were used in both groups, and mild hypothermia was only applied to the treatment grou p.The neural function deficiency scores and the serum activity of SOD and conten t of MDA were compared before and 14 days after treatment. RESULTS:The neurologic impairment scores in the treatment group(13.67±6.24) w ere obviously lower than those in the control group(18.64±8.67)(P< 0.05).The se rum activity of SOD in the treatment group[(1.91±0.26) nkat/L] was markedly inc reased compared with that in the control group[(1.56±0.24) nkat/L](P< 0.01),and the content of MDA in the treatment group[(9.54±1.42) μmol/L] was significant ly lower than that in the control group[(11.46±1.63) μmol/L](P< 0.01). CONCLUSION:MHT can obviously improve the neurological function of patients wit h severe cerebral infarction,and it is related to the elimination of free radica ls.
