MTT药敏试验在老年急性白血病化疗中的应用

摘　　要：目的　探讨老年急性白血病治疗个体化化疗方案。方法　从 2 1例老年急性白血病患者的外周血或骨髓中分离出白血病细胞进行体外原代培养 ,用MTT法检测柔红霉素、高三尖杉酯碱、阿克拉霉素、米托蒽醌、阿糖胞苷、足叶乙苷、鬼臼噻吩甙、羟基喜树碱、甲氨喋呤等九种单药药敏试验 ,并参照药敏试验制定化疗方案 ,进行化疗。结果　 2 1例患者中有一种或一种以上药物敏感者 16例占 76 2 % ,九种药物均耐药者5例占 2 3 8%。 16例接受药敏指导下化疗 ,6例完全缓解 (CR) ,CR率 37 5 % ,4例部分缓解 (PR) ,PR率2 5 % ,有效率 6 2 5 %。总符合率 81 3% ,阳性符合率 84 6 % ,阴性符合率 6 6 7%。结论　MTT药敏试验对于老年急性白血病选择个体化的化疗方案有指导意义。Objective To look for individual chemotherapy program of aged acute leukemia patients.Methods We collected leukemia cells from blood or bone marrow of 21 cases of aged acute leukemia patients,primarily cultured them.Then we tested their sensititvity to drugs of Daunorubicin,Harringtonine,Aclacinomycin,Mitoxantrone,Cytarabine,Etoposide,Teniposide,Hydroxycampoothecin,Methorexate by MTT assay.The outcome were referenced to formulate chemotherapy program.Results Among the 21 cases,16 cases were chemosensitive to one of those drugs or more(76.2%),5 cases were drug resistance which account for 23.8%, 16 cases were treated by those reference programs,6 cases recovered completely (6/16,37.5%),4 cases recovered partly (4/16,25%).The total effective rate was 62.5%.Positive drug sensitive test result were followed better clinical treatment effect,the coincident rate was 84.6%.Negative drug sensitive test result had 66.7% clinical effect coincident rate.The total coincident rate was 81.3%. Conclusion Drug sensitive test of MTT assay has guidance value for individual chemotherapy program of aged acute leukemia.
