胃黏膜癌变过程中细胞周期调控基因Rb、Cyclin D1及CDK4的表达及其意义

摘　　要：目的　研究胃黏膜癌变过程中细胞周期调控基因Rb、CyclinD1及CDK4的表达及其意义。方法　采用免疫组化方法对 5 0例胃癌 ,1 8例癌旁肠化生 ,7例癌旁轻中度不典型增生及 9例重度不典型增生组织中细胞周期调控基因Rb、CyclinD1及CDK4的表达情况进行检测和比较。结果　Rb基因的过度表达在不典型增生及胃癌中检测到 ,在胃癌组织中的表达强度明显大于重度不典型增生病变 (P <0 .0 5 ) ,CyclinD1及CDK4的表达仅发现在胃癌组织中 ,胃癌组织中CyclinD1表达阳性的 1 3例病例中有 5例同时表达Rb ,2例同时表达CDK4 ;Rb阳性的 1 7例胃癌组织中有 3例同时表达CDK4。结论　Rb ,cyclinD1和CDK4的过度表达可能参与胃癌的发生 ,Rb的过度表达出现在胃黏膜癌变过程的较早期阶段 。Objective To investigate the significance of the expression of Rb,Cyclin D1 and CDK4 regulator of cell cycle during the carcinogenesis of gastric mucosa.Methods Immunohistochemical staining technique was used to observe Rb,Cyclin D1 and CDK4 regulators of cell cycle expression in 50 cases of gastric carcinoma,of which 18 intestinal metaplasia(IM),7 mild/moderate atypical dysplasia(Dys) and 9 severe Dys(mucosa adjacent to the tumor).Results Rb over expression was found in mild/middle Dys,severe Dys and gastric carcinoma.The increasing frequency of immunoactivity was found in the gastric carcinoma compared with severe Dys( P <0.05),cyclin D1 and CDK4 protain was found only in gastric cancer.Thirteen of Cyclin D1 positive gastric cancer showed five cases of CDK4 positive and two cases of Rb simultaneity.Seventeen of Rb positive gastric cancinoma showed 3 cases of CDK4 positive simultaneity.Conclusions Overexpression of Rb,Cyclin D1 and CDK4 may involve in the onco pathogenesis of gastric carcinoma.Overexpression of Rb was characteristic of the early phase of gastric carcinogenesis.Cyclin D1 and CDK4 were a late event in gastric carcinogenesis.
