摘  要:目的 探讨非接触眼压 (NCT)对青少年近视患者测定的影响因素及误差分析。方法 本院青少年近视门诊患者共 1 1 7例 (2 34只眼 ) ,年龄 7~ 1 8岁 ,屈光度 - 0 .5 0 D~ - 7.2 5 D,眼压 (NCT) 1 1~ 2 9mm Hg。根据 NCT>2 2 m m Hg及≤ 2 2 mm Hg分为二组 ,并将二组 NCT与角膜厚度、眼轴、屈光度、年龄、性别进行统计分析。其中 39例 NCT>2 2 m m Hg患者均给予压平眼压计测定 ,并对 9例压平眼压偏高、5例眼底视乳头生理凹陷 >0 .5的患者进行长期随访。结果 青少年近视患者的 NCT随屈光芳、眼轴长度的增加而增高 ,二者有显著相关性 (P <0 .0 1 ) ;NCT的高低与角膜厚度高低有显著相关性 (P <0 .0 1 ) ;NCT与年龄、性别无显著相关性。NCT测定还受许多干扰因素影响。 39例 (78只眼 ) NCT>2 2 mm Hg患者给予压平眼压计测定 ,平均压平眼压 (1 9.2 5± 2 .6 3) mm Hg;9例 (1 8只眼 )压平眼压 >2 2 mm Hg、5例眼底视乳头生理凹陷 >0 .5的患者进行二年以上随访 ,无 1例转变成青光眼。结论 影响青少年近视非接触眼压测定值的主要因素是角膜厚度、眼轴长度及屈光度。非接触眼压对近视尤其是高度近视的测量有一定的局限性 ,正确性较差。遇到高眼压先要排除使眼压假性增高的因素 ,有条件给予压平眼压计测定 。Objective To evaluate the intraocular pressure (IOP) error in a population with juvenile myopia as measured by means of a noncontact tonometer(NCT).Methods IOP values of 117 out patient myopia patients (234 eyes),measured by means of NCT were divided into 2 groups with IOP above and below 22 mmHg.Five consecutive measurements were taken in each subject (CCT;length of ocular exis,diopter,age,sex),39 patients with IOP above 22mmHg were tested with Goldmann applanation tonometry.9 patients with Goldmann applanation tonometry results larger than 22mmHg and 5 patients with C/D>0.5 had a long time follow up.Results The positive correlation of IOP to CCT,length of ocular axis and diopter were found (P<0.01) in juvenile myopia.A negative correlation was figured out among IOP,age and sex.Conclusion The main factors affect the IOP of juvenile myopia measured by NCT were CCT,length of ocular axis and diopter.NCT test has some limitation when met myopia larger than 6.00D If necessary,Goldmann applanation tonometry should be used.
