摘　　要：目的　研究分析老年多原发癌患者首癌和次癌肿瘤组织Igκ和Igλ表达及特点。方法　 2 1例长期随访的多原发癌病人 ,首癌为消化道癌的多原发癌者 2 0例 ,首癌与次癌均为消化道癌者 1 7例。应用鼠抗人κ轻链单克隆抗体和鼠抗人λ轻链抗体对肿瘤组织行免疫组化染色。结果　首癌或次癌有Igκ和 (或 )Igλ阳性表达者共 1 6例 ,占 76 2 %。统计学分析表明 ,多原发癌首癌与次癌肿瘤组织的Igκ、Igλ表达关系密切 ,具有明显的相关性(P <0 0 1 )。首癌与次癌Igκ、Igλ表达情况与多原发癌生存率统计分析提示 ,首癌与次癌Igλ同时阴性组与首癌Igλ阴性次癌Igλ阳性组比较 ,后者的次癌生存时间明显缩短 (P <0 0 5 )。结论　Ig样蛋白表达是多原发癌的癌变过程中较为普遍的现象 ,Igκ和Igλ表达在多原发癌的发生发展中可能起重要作用。与首癌、次癌Igλ表达均阴性的多原发癌病人比较 ,如在次癌肿瘤组织中出现Igλ表达阳性 ,预示此类多原发癌患者的生存时间短 。Objective To investigate the abnormal expressions of light chain immunoglobulins kappa (Igκ) and lambda ( Igλ) in multiple primary malignant tumors (MPMT) in elderly patients. Methods The expressions of Igκ and Igλ in specimens obtained from multiple malignant tumors in 21 elderly patients were investigated. Immunohistochemical staining was performed with mouse antihuman Igκ and Igλ monoclonal antibodies. Among 21 patients with multiple primary malignant tumors, the first tumor which occurred in the digestive tract was detected in 20 patients. In 17 patients first and second tumors were found to occur in the digestive tract. In 21 patients with MPMT the relationship between the expressions of light chain immunogloblin and survival time was analyzed. Results Positive staining of Igκ and/or Igλ in specimens from the first and/or second tumor tissues was found in 16 patients(76 2%). Statistic analysis showed that there was close relationship between the expression of Igκ and that of Igλ in the tissues from double tumors ( P <0 01). The survival time in patients with MPMT with negative expression of Igλ in both malignancies was significantly onger then those with negative expression between Igλ negative staining group from double tumor of Igλ but positive expression of Igλ in the second tumor. Conclusions Expressions of light chain immunoglobulin Igκ and Igλ might play a crucial role in the development of multiple primary malignant tumors. A pssitive expression of Igλ in the second tumor might portend a poor prognosis and a shorter surrival time
