摘 要:目的:银杏叶提取物注射液已广泛用于心脑血管病的治疗,观察其对急性脑梗死患者保护脑细胞的作用和自由基的影响,探讨其可能机制。方法:泸州医学院附属医院神经内科收治急性脑梗死56例,诊断符合第四届全国脑血管病学术会议制定的诊断标准,均在发病1~72h内就诊,并经头颅CT证实,梗死灶体积≤10cm3,排除了脑梗死性出血。将患者随机分为治疗组和对照组组各28例,治疗组给予银杏叶提取物治疗,对照组予以复方丹参注射液,并在治疗前及治疗后14d进行神经功能缺损评分和血液流变学指标、血清超氧化物歧化酶(superoxidedismu-tase,SOD)和丙二醛的测定。结果:治疗后治疗组神经功能缺损评分为13.69±7.03,对照组为19.87±9.42,两组比较差异有非常显著性意义(P<0.01)。治疗后两组血液流变学各指标均较治疗前好转,但治疗组更明显,差异有非常显著性意义(P<0.01),血清SOD活性明显升高,丙二醛水平显著下降(P<0.01),未发现明显副反应。结论:银杏叶提取物对急性脑梗死患者脑功能的保护机制可能与改善血液流变学和清除自由基有关。AIM:Ginkgo leaf extract has been extensively used in treating cerebrovascular disease.The aim of this study is to observe the effect of ginkgo leaf extract on the protection of brain cells and the free radical in patients with acute cerebral infarction(ACI),so as to explore the possible mechanisms. METHODS:A total of 56 ACI patients were selected from the Department of Neurology of Affiliated Hospital of Luzhou Medical College.All the patients were in accordance with the diagnostic criteria of the Fourth National Academic Meeting for Cerebrovascular Disease,received treatment 1-72 hours after attack,and with the infarction size≤10 cm3 confirmed by cranial CT,while those with cerebral hemorrhage were excluded.All the patients were divided into treatment group(n=28) and control group(n=28),and they were treated with ginkgo leaf extract and fufang danshen injection respectively.The neurological impairment scores and the hemorheological indexes,the serum activity of superoxide dismutase(SOD) and content of malondiadehyde(MDA) were measured before and 14 days after treatment. RESULTS:The neurological impairment scores were very significantly different between the treatment group(13.69±7.03) and the control group(19.87±9.42)(P< 0.01).The hemorheologcial indexes were improved after treatment in both groups,especially in the treatment group,and there were very significant differences(P< 0.01).The serum activity of SOD was obviously increased,while the serum content of MDA was significantly decreased in the treatment group(P< 0.01),and no obvious side effects were observed. CONCLUSION:The mechanism of ginkgo leaf extract for protecting brain function in ACI patients may be related with the improvement of the hemorrheology and the elimination of free radicals.
