血清γ-谷氨酰转移酶同工酶测定在肝癌鉴别诊断中的意义

摘　　要：目的:建立免疫沉淀法测定血清中低密度脂蛋白(LDL)-极低密度脂蛋白(VLDL)-γ-谷氨酰转移酶(GGT)活性及用垂直板不连续梯度聚丙烯酰胺凝胶电泳分析GGT同工酶,观察其在肝癌鉴别诊断中的价值。方法:以载脂蛋白B(ApoB)抗体沉淀血清中与LDL及VLDL结合的GGT并测定其活性,同时用电泳法分析原发性肝癌、肝硬化、慢性肝炎患者及正常人血清GGT同工酶。结果:血清LDL-VLDL-GGT活性对肝癌的诊断敏感度可达86.2%,肝癌与肝硬化和慢性肝炎鉴别诊断的特异度分别为67.9%和81.3%;GGT同工酶Ⅱ在原发性肝癌患者血清中的阳性率为78.7%,胆石症、肝硬化、肝炎患者的GGTⅡ阳性率分别为15.4%、9.8%和6.4%,正常人为0%。结论:血清LDL-VLDL-GGT活性对鉴别肝癌与其他肝病具有重要意义;血清GGTⅡ是肝癌特异性的同工酶形式。Objective To assess the low density lipoprotein(LDL)-very low density lipoprotein(VLDL)-gamma-glutamyltransferase(GGT) activity by the immunoprecipitation method and analyze the serum gamma-glutamyltransferase isoenzymes by the vertical slab electrophoresis on the non-gradient polyacrylamide gel, in order to estimate the value of serum GGT isoenzymes in diagnosing the liver malignancy. Methods The serum GGT isoenzymes in the patients with liver malignamcy,cirrohosis or chronic hepatitis and the healthy individuals were analyzed by the electrophoresis method using the apolipoprotein (Apo) B antibody as a precipitant to precipitate the parts of GGT which was combined with LDL and VLDL. Results The diagnostic sensitivity of the serum LDL-VLDL-GGT activity for liver malignancies was 86.2%,and the specificity for discriminating liver malignancy from cirrhosis and chronic hepatitis was 67.9% and 81.3% respectively. The positive rate of the serum GGT isoenzymeⅡ in the patients with liver malignancies was 78.7%, which was higher than that in the patients with cholecystolithiasis(15.4%), hepatocirrhosis (9.8%) and hepatitis (6.4%). Conclusions The serum LDL-VLDL-GGT activity has important significance in the diagnosis of the liver malignancy. The serum GGTⅡis a specific isoenzyme for liver malignancy.
