脑干听觉诱发电位对腭裂儿术前听力的评价

摘  要:目的 :探讨腭裂患儿术前的听力受损程度、性质及其脑干听觉诱发电位 (BAEP)的变化与临床意义。方法 :对 32例腭裂儿行脑干听觉诱发电位检测观察其听觉阈值及潜伏期、峰间期的异常率。并与本院保健门诊体检的同龄健康儿进行比较。结果 :195 .31%的腭裂患儿存在听力障碍 ,大部分为中重度损伤 ,其听觉阈值及各波潜伏期较正常儿显著提高和延长 (P<0 .0 1) ,而各波峰间期无显著性差异 (P>0 .0 5 )。 2不同类型腭裂患儿的听力障碍发生率及程度无显著性差异 (P>0 .0 5 )。结论 :腭裂儿存在高比率脑干听觉传导通路障碍 ,主要是传导性听力丧失 ,BAEP检查能早期、准确了解其听力情况 ,作为筛选适于低龄儿听力的工具 ,具有重要的临床意义。Objective:To explore auditory lesion degree in children with cleft palate before operation and the changes and its clinical significance of BAEP. Method:Thirty-two children with cleft palate were examined with the BEAP . The auditory thresholds,the peak latency(PL)and the interpeak latency(IPL)were measured and compared with 30 healthy children of the same age group.Results:①95.31% children with cleft palate had a hearing defect ranging from middle to severe abnormality.The auditory thresholds of the children with cleft palate were higher than those of the normal children and the PL were longer than those of the normals(P<0.01).There was no significant difference among every IPL(P>0.05) ②There was no significant difference of the hearing defect occur ration and degree in the children with cleft palate of different type (P>0.05).Conclusion:Children with cleft palate had hearing loss,of which most were conductive hearing loss .The BAEP as a method of hearing examination for young children ,was of significant clinical importance.
