摘  要:【目的】 分析超重及肥胖对儿童血压、血脂和空腹胰岛素水平的影响。 【方法】 在北京城区 4所小学中随机选出 3 17名 8~ 11岁健康儿童进行体格、血压测量及血脂和胰岛素检测。 【结果】 超重肥胖儿童的检出率为 2 4% ,在对年龄性别修正后 ,超重肥胖儿童与正常儿童相比血压、胰岛素、甘油三脂、总胆固醇及脂蛋白 (a)显著升高 ,高密度脂蛋白 /低密度脂蛋白、载脂蛋白A/载脂蛋白B显著降低 ,逐步回归分析显示体质指数与收缩压、甘油三脂及胰岛素呈正相关关系 ,与高密度脂蛋白呈负相关关系。在对体重指数修正后 ,胰岛素与甘油三脂及收缩压的偏相关系数分别为 0 .2 0 (P <0 .0 1)和 0 .12 (P <0 .0 5 )。 【结论】 肥胖可引起儿童脂代谢紊乱和高血压 ,且胰岛素敏感性的改变可能在这一病理过程中起重要作用。Objective To analyze the effect of overweight on blood pressure, lipids and insulin in children. 317 healthy children aged 8~11 years were selected randomly from 4 primary schools in Beijing. Fasting insulin and lipids were measured and anthropometry was carried out for all samples. Percent 24 of the children were overweight. After adjusting for age and sex, the blood pressure and the levels of fasting insulin, cholesterol and triglyceride were significantly higher (P<0.01) , and the ratios of high-density lipoprotein(HDL)/low-density lipoprotein (LDL) and apoA/apoB were significantly lower (P<0.01) in the overweight children than those in the ideal weight children. Multiple linear regression analysis showed that body mass index (BMI) was positive correlation with systolic pressure, fasting insulin and triglyceride, and negative correlation with HDL. After controlling BMI, the partial correlation coefficients between fasting insulin and triglyceride, systolic pressure were 0.20 (P<0.01) and 0.12(P<0.01) respectively. [Conclusion] Overweight may be one of the reasons for the metabolic disorders of lipids and hypertension in children, and higher serum insulin may contribute to the metabolic disorders in overweight children.
