摘　　要：【目的】寻求预防膀胱癌手术后复发的更有效方法。【方法】62例膀胱癌患者随机分为两组:Ⅰ组 34例,采用手术中THP+IL 2膀胱切缘黏膜下注射和术后灌注THP、IL 2及给予华蟾素治疗;Ⅱ组28例, 应用术中THP+IL 2膀胱切缘黏膜下注射和术后灌注THP、IL 2;分别观察两组的疗效和副作用。【结果】 平均随访27(16～35)个月。Ⅰ组34例患者中复发率为5.88%(2/34);未见明显局部或全身副反应。Ⅱ组复 发率为25.00%(7/28),有3例出现膀胱刺激症状,2例有疲倦症状,2例出现恶心,1例出现呕吐。两组肿瘤 复发率差别有显著性(P<0.05)。【结论】采用手术中THP+IL 2膀胱切缘黏膜下注射和术后灌注THP、 IL 2及给予华蟾素治疗预防膀胱癌术后复发疗效好、副反应少,值得临床推广应用。ObjectiveTo find out the more effective route for preventing postoperative recurrence of bladder cancer (BC).Sixty-two patients with BC were randomly divided into two groups: ①Group Ⅰ,34 patients were treated with submucous injection of pirarubicin (THP) plus interleukin-2 (IL-2) along the incised edge of bladder during operation , postoperative intravesical infusion of THP plus IL-2 and iv drip of Chinese traditional medicine(cinobufacin); ②Group Ⅱ, the remaining 28 patients were treated with submucous injection of THP plus IL-2 along the incised edge of bladder during operation and postoperative intravesical infusion of THP plus IL-2.The therapeutic efficacy and adverse effects in both groups were observed respectively.Patients were followed-up for 16~35 months (averagely 27 months).The recurrence rate in 34 patients of Group Ⅰ was 5.88% (2/34), without obvious local and systemic adverse effects. Among 28 patients in Group Ⅱ, the recurrence rate was 25.00%(7/28), with symptom of bladder irritation in 3 cases, fatigue in 2 cases, nausea in 2 cases and vomiting in 1 case. Significant difference in cancer recurrence rate was found between both groups (P<0.05).[Conclusions]Submucous injection of THP plus IL-2 along the incised edge of bladder during operation and postoperative intravesical infusion of THP plus IL-2 combined with cinobufacini iv drip after operation are a safe and effective therapy for preventing postoperative recurrence of BC, with less adverse effects. So it is worthy to be widely used clinically.
