肺癌伴恶性胸水的外科及过继免疫治疗

摘　　要：目的探讨手术配合香菇多糖免疫法治疗非小细胞肺癌伴恶性胸水患者的疗效。方法60例非小细胞肺癌伴恶性胸水患者按配对的原则分为联合治疗组(手术+香菇多糖)30例,单纯手术组30例;观察患者近期疗效、生活质量及免疫学指标等的变化。结果联合治疗组与单纯手术组近期有效率分别为80.0%,46.7%(P<0.05)。生活质量联合治疗组与单纯手术组总提高率分别为83.3%,46.7%(P<0.05)。两组治疗前T淋巴细胞亚群比较无显著性差异,联合治疗组治疗后1周与治疗前比较,CD4+和CD4+/CD8+比值改善(P<0.05),且优于单纯手术组(P<0.05)。结论采取外科手术与香菇多糖过继免疫联合治疗,能提高肺癌伴恶性胸水患者的生活质量并能改善其免疫功能。Objective To explore the effect of lentinan combined with surgical treatment on the non-small cell lung cancer with malignant pleural effusion.Methods 60 cases of non-small cell lung cancer companied with malignant pleural effusion were divided into combination treatment group(surgical treatment +lentinan)(n=30) and surgical group(n=30) for the observation of short-term effect,life quality and immunity indexes.Results The short-term effective rate was 80.0% and 46.7%,respectively in combination treatment group and surgical group^(P<0.05) .The improvement rate of life qualtiy was 83.3% and 46.7%,respectively in combination treatment group and surgical group(P<0.05).CD4+ and CD4+/CD8+ in combination treatment group after 1 week treatment were improved but there was no significant difference in T-lymphocyte before treatment in both groups,which was superior to surgical group(P<0.05).Conclusion Surgical treatment combined with lentinan can improve the life quality and immunity function of the patients with lung cancer companied with malignant pleural effusion.
