恶性消化系肿瘤晚期患者空腹和服糖后血糖与乳酸变化及其意义

摘　　要：目的　观察恶性消化系肿瘤晚期患者空腹和服糖后血糖及乳酸变化,探讨消化系恶性肿瘤晚期患者糖代谢 紊乱表现及其机理。方法　测定45例恶性消化系肿瘤晚期患者和45例健康体检正常人员空腹血糖、空腹乳酸、 服糖2h后血糖和乳酸含量,并计算服糖2h后血糖和乳酸含量与空腹血糖和乳酸含量比值。结果　恶性肿瘤晚 期患者空腹乳酸、服糖2h后血糖和乳酸含量均明显高于正常对照组(P<0.01);服糖2h后血糖与空腹血糖比值 以及服糖2h后乳酸与空腹乳酸比值,明显高于正常对照组(P<0.01)。恶性消化系肿瘤晚期患者空腹血糖与正 常对照组无显著性差异(P>0.05)。结论　恶性消化系肿瘤晚期患者尽管空腹血糖正常,但仍存在糖代谢紊乱。 糖代谢紊乱主要表现为空腹高乳酸血症和服糖后高血糖和高乳酸反应。Objective To study the changes of the fasting and postprandial plasma glucose levels and lactic acid anctivity in the patients with advanced maligrant gastrointestinal tumors,and the mechanism of disorder of glucose-metabolism on this states.Methods The fasting and postprandial plasma glucose level and lactic acid activity in 45 cases of the above diseases were measured and the ratios of postprandia plasma glucose level over fasting one and lactic acid activity were calculated.Results The levels of the fasting and the postprandial plasma glucose levels and lactic acid in the patients with were significantly higher than that in the control(P< 0.01 );The levels of the ratios of postprandial plasma glucose and fasting plasma glucose and the ratio of postprandial plasma lactic acid and fasting plasma lactic acid in the patients were significantly higher than that in the control(P<0.01).There was no significant difference of the levels of fasting plasma glucose between the patients and the controls (P>0.05).Conclusion The disorder of glucose-metabolism of the patients with advanced maligrant gastrointestinal tumors represent a postprandial glycemia and high level of lactic acid.The fasting glucose leve is not always higher than that of the control in this states.
