胃B细胞淋巴瘤API2-MALT1融合基因表达与幽门螺杆菌感染的相关性

摘　　要：目的　探讨胃MALT淋巴瘤和弥漫性大B细胞淋巴瘤演进中t(11;18)(q21;q21)与幽门螺杆菌(HP)感染的关系, 以及检测API2-MALT1融合基因的意义。方法　47例胃淋巴瘤病例(MALT淋巴瘤31例,弥漫性大B细胞淋巴瘤16例),经 复查诊断后,用RT PCR和巢式PCR,检测肿瘤组织中API2-MALT1融合基因的表达,用半巢式PCR和特殊染色检查HP感染 情况。根据淋巴瘤类型及融合基因检测结果将病例分组,观察各组病例HP感染的差异。结果　47例胃淋巴瘤中20例API2 - MALT1融合基因mRNA检测阳性,包括16例MALT淋巴瘤和4例弥漫大B细胞淋巴瘤。HP感染检出率:API2- MALT1阳性 组为25%(5/20),API2- MALT1阴性组25.92%(7/27)。统计学分析表明两组间差异无显著性。2例API2 MALT1融合基因 检测阴性的胃MALT淋巴瘤病例经抗HP治疗后病情稳定。结论　胃MALT淋巴瘤和DLBCL中API2- MALT1mRNA表达与 胃淋巴瘤HP感染无明显相关,API2- MALT1融合基因可能成为胃MALT淋巴瘤抗HP治疗效果不良的预测因子。Purpose To investigate the correlation between the expression of API2-MALT1 fusion mRNA and infection of helicobacter pylori (HP) and understand significance of detecting the fusion transcript in the progression of gastric B-cell lymphomas. Methods Forty-seven cases of gastric B-cell lymphomas (MALT lymphoma 31,diffuse large B-cell lymphoma 16) were collected and retrieved. API2-MALT1 transcripts were detected by RT-PCR and nested PCR,and the infection of HP were examined by a dyeing of methylene blue and semi-nested PCR. All cases were grouped based on whether or not expressing API2-MALT1 fusion mRNA and the type of lymphoma,then the infection of HP was compared among the groups. Results API2-MALT1 fusion transcripts were detected in 20 of 47 cases with gastric B-cell lymphoma,including 16 cases of MALT lymphoma and 4 cases of diffuse large B-cell lymphoma. The frequency of infection of HP was 25%(5/20)and 25.92%(7/27) respectively in the group of API2-MALT1+ and API2-MALT1-. The infectious rate was similar between the two groups. Two of MALT lymphoma cases,with the infection of HP but without the expression of API2-MALT1 fusion,were found to have a good response to the treatment of antibiotic. Conclusions It is not ensured that the correlation between the expression of API2-MALT1 fusion mRNA and infection of HP in gastric B-cell lymphomas. API2-MALT1 is likely to be a marker for gastric MALT lymphoma that will not response to HP eradication.
