中国人胃癌nm23H1基因遗传不稳定性与临床病理特性的相关性

摘　　要：目的　研究17号染色体D17S3 96位点微卫星不稳定性和杂合性缺失对胃癌nm 2 3H1蛋白表达的影响,为胃癌临床治疗及分析预后提供实验依据。方法　在石蜡包埋组织中抽提DNA ,应用PCR 单链构象多态性(SSCP) ,常规银染、Envision免疫组织化学及Leica Qwin计算机图像分析等方法进行nm 2 3H1基因遗传不稳定性研究。结果　40例胃癌D 17S3 96位点MSI、LOH检出率和nm 2 3H1蛋白阳性率分别为2 0 .0 0 % ,17.5 0 % ,5 5 .0 0 %。TNMⅠ+Ⅱ期的MSI检出率为3 1.82 % ,显著高于Ⅲ+Ⅳ期(5 .5 6% ) ,而LOH则相反。淋巴结转移组的MSI检出率为5 .0 0 % ,显著低于无淋巴结转移组(3 5 .0 0 % ) ,而LOH则相反。nm 2 3H1蛋白阳性率在淋巴结转移显著低于无淋巴结转移。结论　MSI和LOH通过不同的途径调控散发性胃癌的发生和转移。MSI是胃癌的早期分子指标之一;LOH可与胃癌高淋巴结转移、预后差的特性相关。Objective To research the effect of microsatellite instability(MSI) and loss of heterozygosity(LOH) of locus D17S396 on the expression of gene nm23H 1 in gastric cancer and to provide experimental basis for clinicaltreatment and prognosis.Methods Techniques such as DNA extraction from paraffin-embedded tissues,polymerase chain reaction-single strand conformation polymorphism(PCR-SSCP),ordinary silver stain,Envision immunohistochemistry and Leica-Qwin computer imaging techniques were used to investigate the genetic instability of gene nm23H 1.Results In the experiments,the frequency of MSI,LOH and nm23H 1 protein expression of 40 cases of gastric cancer were 20.00%,17.50% and 55.00% respectively.Based on the TNM staging,the frequency of MSI was higher in stage Ⅰ+Ⅱ disease(31.82%) than in stage Ⅲ+Ⅳ disease(5.56%),while LOH was on the contrary.The frequency of MSI was significantly lower in cases with lymph nodes metastases(5.00%) than in cases without (35.00%),while LOH was on the contrary.The expression of nm23H 1 protein was significantly lower in cases with lymph nodes metastases than in cases without.Conclusion MSI and LOH can controll the carcinogenesis and metastasis of sporadic gastric cancer through different approaches.MSI is an molecule marker in the early period of sporadic gastric cancer.On the contrary,LOH appears mostly in the late period of sporadic gastric cancer,indicating a high metastsis rate and poor prognosis.
