利培酮氯丙嗪对首发精神分裂症患者认知功能及事件相关电位P_(300)的影响

摘　　要：目的比较利培酮和氯丙嗪对首发精神分裂症患者认知功能的影响.方法采用随机对照研究连续观察12周,利培酮组33例,剂量范围为1～7mg/d;氯丙嗪组22例,剂量范围25～600mg/d.进行PANSS、韦氏记忆量表(WMS)和事件相关电位P300的测评.结果首发精神分裂症患者在长时记忆、短时记忆、瞬时记忆及记忆商数(MQ)受损较为明显,与对照组比较差异有显著性(P＜0.05).P300电位成分中P2[(186.1±22.8)ms]、N2[(241.0±24.5)ms]及P3[(349.1±28.2)ms]潜伏期明显延长,P2[(3.7±2.1)μV]及P3[(4.2±2.5)μV]波幅明显降低,与对照组比差异均有显著性(P＜0.05).经过12周治疗利培酮治疗后WMS的再认、联想、理解、背数及记忆商(MQ)明显高于氯丙嗪,两组在治疗前后P300各指标之间无显著差异.结论首发精神分裂症患者存在着认知功能障碍,利培酮对首发精神分裂症认知功能的影响明显好于氯丙嗪.ObjectiveTo compare the effects of risperidone and chlorpromazine on cognitive function in first-episode schizophrenic patients.MethodsA randomized comparative clinical study was used to compare the cognitive function of risperidone and chlorpromazine before and after 12-week treatment.55 patients with first-episode schizophrenia were divided two groups . 33 cases were treated with risperidone in a regimen of 1~7mg/d.22 cases were treated with chlorpromazine in a regimen of 25~600mg/d. PANSS 、WMS and P 300 were used to evaluate adverse effects of drugs and cognitive function.ResultsThe long-term memory,short-term memory, immediate memory, MQ of first-episode schizophrenia were damaged obviously. Latency periods of P 2 [(186.1±22.8) ms],N 2 [(241.0±24.5) ms] and P 3 [(349.1±28.2) ms]of P 300 potentials with first-episode schizophrenia group was longer than those of the control group, the amplitude of P 2 [(3.7±2.1)μV] and P 3 [(4.2±2.5)μV]of P 300 potentials with first-episode schizophrenia group was lower than that of the control group. There were significant difference between the two groups (P<0.05 ) . The recognize,association,comprehend,recite number ,MQ of WMS were significantly higher in risperidone than in chlorpromazine, while no differences in P 300 between two groups before and after drug treatment.ConclusionThe presence of cognitive dysfunction in first-episode schizophrenia has been confirmed .The study suggests that the effects of risperidone on cognitive function are better than that of chlorpromazine.
