摘 要:目的:分析认知、适应测验/临床语言、听力里程量表项目的难度及鉴别力。方法:于2003-12/2004-06以1907名上海市足月且出生体质量>2500g的健康婴幼儿为常模,用认知、适应测验/临床语言、听力里程量表进行测试,排除双胎或多胎、产时窒息的婴幼儿。按月龄1~12,14,16,18,21,24,30,36个月分为19个年龄组,认知、适应测验/临床语言、听力里程量表项目中主观性项目通过询问家长获得答案,客观性项目当场测试,由测试人员判断项目通过与否给予评分。然后计算每个年龄组项目在相应年龄组儿童中的通过率及变差,并对每个项目的得分与测验总分的相关性进行分析。依据通过率及变差判定项目的难易和鉴别力。当通过率为0.50时,变差为0.5,项目的鉴别力最高,变差越接近0.5,表明鉴别力越好。结果:①临床语言、听力里程量表项目的平均通过率为0.71、变差为0.38。项目难度合适占84%(36/43),项目过易占14%(6/43),项目过难占2%(1/43);每个项目的得分与测验总分相关系数为0.11-0.86,平均为0.68(经t检验,P均<0.0001)。②认知、适应测验项目的平均通过率为0.66、变差为0.42,项目难度合适占93%(53/57),项目过易占7%(4/57),无过难项目;各项目的得分均与测验总分相关系数为0.17-0.86,平均为0.67,(经t检验,P均<0.0001)。③认知、适应测?AIM:To analyze the difficulty and the discriminating capability of the items in the cognitive adaptive test/clinical linguistic and auditory milestone scale(CAT/CLAMS). METHODS:From December 2003 to June 2004,CAT/CLAMS was employed to assess 1 907 normative full-term children with body mass more than 2 500 g at 1 month to 36 months of age in Shanghai,excluding twins or multiple birth and intrapartum asphyxia children.The children were divided into 19 age groups of 1 to 12 months,14,16,18,21,24,30,36 months according to their months old.Subjective CAT/CLAMS items were acquired by inquiring the children's parents,and the objective CAT/CLAMS ones were performed with spot test.Then,the tested items were judged and scored by conners.The passing percentage,the variation in the items of each age group for the subjects of the corresponding one as well as Pearson Correlation Coefficients between the scores in each item and the total score of the test were calculated.The difficulty and discriminating capability of the items in CAT/CLAMS were determined based on the tested passing percentage and variation.When the passing percentage was 0.50 and the variation was 0.5,the discriminating capability of the items was the best.The closer the variation was to 0.5,the better was the discriminating capability. RESULTS:①The mean difficulty of the items in CLAMS was 0.71 and the variation was 0.38.The moderate difficulty of items was 84%(36/43),the excessive easiness was 14%(6/43),and the excessive difficulty was 2%(1/43).The score for each item in CLAMS was correlated positively with the total score and the correlation coefficients varied from 0.11 to 0.86 with the mean of 0.68(P< 0.000 1 by t test).②As to CAT,the mean difficulty was 0.66 and the variation was 0.42;The moderate difficulty of items was 93%(53/57),the excessive easiness was 7%(4/57),and the excessive difficulty was 0.The score for each item in CAT was correlated positively with the total score and the correlation coefficients varied from 0.17 to 0.86 with the me
