摘 要:目的射频消融灶影像学及病理表现以及注射盐水对消融范围的影响。方法超声引导下经皮穿刺实验猪肝脏进行射频消融,温度100℃,时间20min。消融术后即刻进行增强CT扫描,术后饲养不同时间并复查CT,处死后行病理检查。结果100℃温度下,消融灶最大径3.16cm,注射盐水后最大径4.08cm,二者差别具显著统计学意义。CT随访显示消融灶不同程度缩小。病理切片消融灶内呈凝固性坏死,CT显示为无增强病灶。结论射频消融在正常动物肝脏中可产生有效的凝固坏死灶。注射盐水可明显增加消融灶范围。Objective To investigate the radiological and pathological features of ablated lesions obtained by the expandable RF system, and the influence of saline injection during RF ablation on lesion size. Methods In vivo radiofrequency ablation (RFA) was processed on 5 normal swine with the 3rd gear of electrode size. The ablation temperature was 100℃ while the treatment time was 20 minutes. RFA with saline injection was performed in 6 lesions and without in 5, resulting in 11 lesions as a whole. RFA lesions were examined by triple-phase contrast-enhanced CT immediately after the procedure and several weeks later on, respectively, and all swine were sacrificed finally for pathological evaluation. Results The mean diameter of RFA lesions obtained with temperature of 100℃ and 3rd gear of electrode's size was 3.16 cm, while that with saline injection was 4.08 cm on post-procedural triple-phase CT scan. Preoperative injection of saline could significantly increase RFA lesion size compared with that of non-injection group (P<0.05), whether on post-procedural CT scan or follow-up. On CT scans during follow-up, the RFA lesions shrunk in various extent correlating with time and fibrous repair. Coagulation necrosis was observed in HE stained slices of RFA lesions which manifested as non-enhanced areas on enhanced CT. Conclusion The used radiofrequency system can result in coagulative necrosis reliably and definitely, and injection of saline may increase the size of coagulated area.
