BCL-2、Ki-67、CyclinD1在弥漫性大B细胞淋巴瘤中表达的临床意义

摘　　要：目的探讨BCL-2、Ki-67、CyclinD1蛋白在弥漫性大B细胞淋巴瘤(DLBCL)中表达的临床意义。方法采用免疫组织化学方法检测DLBCL病例中BC-L2、Ki-67、CyclinD1蛋白的表达,分析它们的表达与DLBCL临床特征、治疗效果及生存期的关系。结果BCL-2、Ki-67、CyclinD1蛋白在DLBCL中表达阳性率分别为48.3%、58.3%、37.9%,它们的表达之间没有相关性;BCL-2、Ki-67、CyclinD1蛋白在DLBCL中的表达与患者的年龄、性别无关,与DLBCL的临床分期、疗效及预后有关,即临床分期为Ⅲ/Ⅳ期、疗效差组上述蛋白表达的阳性率较高;BCL-2、CyclinD1蛋白在有全身症状的DLBCL患者组中的表达阳性率高于无全身症状组;而Ki-67表达在DLBCL患者中与出现全身症状有无无关。结论BCL-2、Ki-67、CyclinD1在DLBCL中的表达与DLBCL的临床特征、治疗效果及预后有关;BCL2、Ki-67、CyclinD1蛋白的检测可以作为DLBCL患者的不良临床特征、治疗效果及不良预后的指标。Objective To investigate the clinical significance of the expression of BCL-2,Ki-67 and Cyclin D1in diffuse large B-cell lymphoma. Methods Immunohistochemical staining was used to detect the expression of BCL-2,Ki-67 and Cyclin D1 in the lymphnode tissues from DLBCL patients and their relation. Results The rate of positivity of BCL-2,Ki-67 and Cyclin D1 proteins in DLBCL patients was 48.3%, 58.3% and 37.9% respectively, with no significant difference between them. The expressions of BCL-2, Ki-67 and Cyclin D1 proteins had relationships with the patients’ Ann arbor stage, chemotherapy efficacy and survival; but not with the patients’ age and gender. Such expressions were higher in DLBCL patients with Ann arbor stage Ⅲ/Ⅳand poor chemotherapy efficacyand survival. DLBCL patients with systemicsymptomsshowed significantly hihger BCL-2 and Cyclin D1 expressions, but no higher Ki-67 expression, than those without systemicsymptoms. Conclusion There arecertain relationshipsbetween the expression of BCL-2, Ki-67 and Cyclin D1 proteins and the clinical features and chemotherapy efficacy, so these expressions can be used as a marker to prognose poor chemotherapy efficacyand survival.
