托吡酯单药治疗各型癫的临床疗效

摘　　要：目的　观察托吡酯(TPM)单药治疗各型癫的疗效,评价个体化用药的方案及安全性。方法　对77例部分性发作或全面性发作的癫疒间患者予TPM单药治疗,成人初始量为2 5mg·d-1,每周增加2 5mg·d-1,达到75～10 0mg·d-1;儿童服用TPM初始量0 5mg·kg-1·d-1,每周增加0 .5mg·kg-1·d-1,达到2～3mg·kg-1·d-1,然后观察疗效,并根据患者发作程度的变化调整维持用药剂量。平均治疗7mo ,对患者在加量期、稳定期发作情况及药物不良反应进行开放性自身对照研究。结果　TPM治疗各型癫疒间总有效率为88% ,其中部分性癫疒间发作的总有效率为86 . 4 % ,全面性发作的总有效率为90 . 3% ,儿童组总有效率为90 . 6 % ,成人组总有效率为86 . 1% ,2组比较,P >0 . 0 5。控制发作的平均用药剂量成人组为(93. 9±17 .2 )mg·d-1,儿童组为(6 3. 8±16 . 2 )mg·d-1。药物不良反应主要为厌食、消瘦、言语减少、记忆减退等。结论　TPM单药能有效控制各型癫发作,成人和儿童疗效无明显差异。AIM To explore the efficacy and safety of topiramate monotherapy on different types epilepsy and to evaluate the dosage-personal therapy trail. METHODS Seventy-seven patients, about 3 to 75 a, who experienced partial-onset or primary generalized seizures were treated with topiramate only. According to each patient’s reaction, the dosage was titrated to seizure-free or seizures obviously reduce. During the titration period and the stabilization period, the rates of seizures and the adverse drug reactions were observed.RESULTS The total efficacy of topiramate monotherapy in different types epilepsy was 88%. The efficacy among in the partial-onset group was 86.4 %, and in the primary generalized seizures group was 90.3%. The average dosage of seizure-free was (93.9±17.2) mg·d -1 in adult, (63.8±16.2) mg·d -1 in children / teenagers. Adverse drug reactions included anorexia, weight loss, difficulty in speak. CONCLUSION Topiramate is effective and well-tolerated as monotherapy in patients(adult or children) with partial-onset or primary generalized seizures.
