摘 要:目的:探讨端粒酶逆转录酶(humantelomerasereversetranscriptases,hTERT)和WT1mRNA在白血病中的表达情况,并分析两者的相关性。方法:使用免疫细胞化学方法检测急、慢性白血病患者骨髓或外周血标本中hTERT的表达,RTPCR方法测定WT1mRNA含量。结果:hTERT在60例急性白血病及7例慢粒加速/急变期(CMLap/bc)患者阳性率分别为81.7%(49/60)和85.7%(6/7),明显高于慢粒慢性期(CMLcp)及正常对照组,P值分别为0.002和0.000。WT1mRNA在急性白血病中呈高表达,阳性率为80.6%。WT1mRNA水平与hTERT阳性细胞百分数间呈轻度负相关,但差异无统计学意义,r=-0.331,P=0.092。hTERT阳性组CR低于阴性组,差异无统计学意义(24%vs66.7%,P=0.067)。WT1mRNA与hTERT均阳性者CR更低,仅11.1%(1/9)。结论:hTERT和WT1mRNA在急性白血病中表达均升高,但两者间未见显著性相关。hTERT与WT1mRNA的联合检测可更好的判断白血病患者的预后。OBJECTIVE:To investigate the expression of human telomerase reverse transcriptases (hTERT) and WT1 mRNA in leukemia cells, and analyze their association.METHODS: hTERT expression in bone marrow or peripheral blood mononuclear cells of patients with leukemia were detected by the immunocytochemical staining, and WT1 mRNA levels were measured by the reverse transcription polymerase chain reaction (RT-PCR) assay.RESULTS: hTERT expressions in 60 patients with acute leukemia and 7 patients with chronic myeloid leukemia at accelerated phase/blast crisis (CML-ap/bc) were 81.7% and 85.7%, respectively, which were significantly higher than those in chronic myeloid leukemia-chronic phase (CML-cp) or normal cases, P=0.002 and 0.000, respectively. The patients with acute leukemia exhibited elevated WT1 mRNA levels. The percentage of hTERT positive cells showed a slightly reverse correlation with the levels of WT1 mRNA( r=-0.331 ), but no statistic significance, P=0.092. The complete remission (CR) rate in hTERT positive group was lower than that in hTERT negative group, but the difference was not significant (24% vs 66.7%, P=0.067). The patients who had both hTERT and WT1 mRNA expressions presented a lower CR rate (only 11.1%). CONCLUSIONS: hTERT expression and WT1 mRNA level are significantly higher in acute leukemia, but no significant correlation is found each other. Combined detection of these two markers may provide a more accurate way to evaluate the prognosis of acute leukemia.
