摘  要:目的研究纳洛酮在急性脑梗塞发生后,对不同时期梗塞病灶密度影(CT值)和周围水肿带面积的影响。方法将72例脑梗塞患者随机分为两组,其中治疗组36例,对照组36例。两组患者均采用传统脑梗塞的治疗方法(抗凝、脱水、抗炎、酌情控制血压)。治疗组在此基础上使用纳洛酮4m g/d治疗。观察纳洛酮在脑梗塞发生后,对梗塞病灶CT值及周围水肿带面积影响的动态变化。结果治疗组患者较对照组患者,梗塞病灶CT值的差异有显著性(P<0.01),第14天后,治疗组CT值显著性上升(P<0.01)。治疗组梗塞病灶周围水肿带面积一直较对照组少(P<0.01),并且治疗组脑梗塞水肿占位面积呈动态减少(P<0.01)。结论纳洛酮对梗塞病灶内脑组织恢复是有帮助的。能减轻梗塞病灶周围脑水肿,从而促进脑神经功能的康复。治疗过程中未见明显副作用出现。Objective To study the effect of nalxone hydrochloride injection on the density (CT value) of the focuses and their surrounding edema area of various periodic cerebral infarction. Methods There were 36 cases in the treatment and controlled groups. Both groups were treated with the traditional methods (contra-coagulation, dehydration,contra-inflammation and properly controlling blood pressure). In addition, the treatment group was given nalxone hydrochloride injection 4mg per day. The dynamic changes of the density of the focuses and their surrounding edema area of various periodic cerebral infarction were observed and compared. Results There were significant difference in the density of the focuses of various periodic cerebral infarction(P〈0.01). The density (CT value) of the focuses showed a significant dynamic increase (P〈0.01). The treatment group was lower than the controlled one in the edema area around the focuses of various periodic cerebral infarction (P〈0.01). And the edema area around the focuses was dynamically alleviated (P〈0.01). Conclusion Nalxone hydroehloride injection was helpful to alleviate the impairment of the edema area around the focuses of cerebral infarction and improve the neurological rehabilitation. And no side effect was found.
