食管鳞状细胞癌外周血单个核细胞Th1/Th2漂移的临床意义

摘　　要：目的:探讨外周血单个核细胞(PBMC)Th1、Th2类细胞因子的表达与食管鳞状细胞癌病理因素之间的相关性。方法:采用双抗体酶联免疫吸附法(ELISA)检测39例食管鳞状细胞癌患者外周血Th1、Th2类细胞因子的含量水平,并与食管鳞状细胞癌病理因素之间进行相关分析。结果:食管鳞状细胞癌患者PBMC中,IL-2、INF-γ的表达明显低于健康对照组,P<0·05;IL-6的表达明显高于对照组,P<0·05,存在Th1/Th2漂移现象。且发现TNM分期越晚,Th1类细胞因子IL-2的表达越弱,而IL-6表达越强,差异有统计学意义,P<0·05。Th1/Th2漂移现象与肿瘤分化程度无明显相关性。结论:食管鳞状细胞癌患者PBMC存在Th2细胞优势,而Th1类细胞因子表达减弱;Th1/Th2漂移现象与肿瘤的TNM分期有关。OBJECTIVE： To study the relationship mononualear cell between the expression of Thl Th2 type cytokincs of peripheral blood and its pathological factors in the patients with esophageal squamous cell cancer. METHODS： Enzyme linked immunosorbent assay （ELISA） was used to detect the expressions of Thl （IL-2 and INF-γ） and Th2 （IL-4,IL-6 and IL-10） type cytokines in and thciv signi fi cances 39 patients with esophageal squamous cell carcinoma. RESULT： Th1 Th2 shift existed in the patients with esophageal squamous cell cancer, levels of Th1 type cytokincs were significantly lower in the patients with esophageal squamous cell cancer than those in the control group, hut Th2 type cytokines （IL-6） was on the contrary. In the advanced patients with esophageal squamous cell cancer, the level of Thl type cytokines （especially IL-2） deeply decreased, but IL-6 was much increased. P〈0.05. Levels of Th1 and Th2 type cytokincs did not show relation with tumor differentiation. CONCLUSION： The level of Th1 type cytokines weakens that of in PBMC of patients with esophageal squamous cell cancer, lint Th2 type cytnkines enhances. The Th1 Th2 shift is associated with TNM stage in patients with esophageal squamnus cell cancer, hut show no relation with tumor differentiation.
