表皮黏钙素基因突变及蛋白表达异常与喉癌转移的关系

摘　　要：目的:探讨表皮黏钙素(E-cd)基因突变及蛋白表达异常在喉癌转移中的作用。方法:采用PCR-SSCP-DNA测序和免疫组化ABC方法,检测64例喉癌组织和9例正常喉组织E-cd基因第5和第7外显子的突变及E-cd蛋白表达。结果:64例喉癌组织中,E-cd基因第5和第7外显子的突变率分别为7.8%(5/64)和9.4%(6/64),基因突变随喉癌临床分期的提高而有增加的趋势,并与淋巴结转移间有密切相关性(P<0.05)。正常喉组织中可见有功能蛋白的表达,在喉癌组织标本中,E-cd呈现异型性表达。染色强度随病理分级提高而减弱趋势。在有淋巴结转移组中,E-cd表达多为阴性或弱阳性,与无转移组比较差异显著(P<0.05)。结论:E-cd基因突变及蛋白表达异常可能在喉癌发生及淋巴结转移过程中起作用。Objective To explore the effects of E-cadherin （E-cd） gene mutation and its abnormal protein expression on the metastasis of laryngeal cancer. Methods Mutations of E-cd gene exon 5, 7 and E-cd protein expressions in 64 cases of laryngeal cancer were detected with the methods of PCR-SSCP-DNA Sequencing and immunohistochemistry. Results E-cd mutation frequency was 17.2%（11/64） in the laryngeal cancer tissues. The E-cd gene mutation was apparently associated with clinical stage and lymph node metastasis （P〈0. 05）. And low expression of E-cd protein was found in laryngeal cancer with metastasis （P〈0. 05）. Conclusion The E-cd gene mutation and abnormal expression of E-cd protein could play a role in the lymph node metastasis of laryngeal cancer by inhibiting the tumor cell adhesiveness and increasing its invasiveness.
