门静脉主支癌栓CD蛋白表达及DNA倍体检测的意义

摘　　要：[目的]通过检测内皮钙黏附蛋白(E-CD)和CD44蛋白的表达水平,检测门静脉癌栓中DNA倍体的比例,探讨其在肝癌肝内转移中的作用。[方法]采用免疫组化方法检测33例门静脉癌栓中E-CD和CD44蛋白的表达,在光镜下对33例门静脉癌栓石蜡标本中肝癌细胞染色体倍数按照超5倍体、2倍体和异倍体进行统计。并与各自原发灶和癌旁组织进行对照。[结果]①E-CD蛋白的表达:门静脉癌栓、肝癌原发灶、癌旁组织的平均吸光度分别为0.015 2±0.003 6,0.052 7±0.007 1和0.100 8±0.006 3。②CD44蛋白的表达:门静脉癌栓、肝癌原发灶、癌旁组织的平均吸光度分别为0.015 9±0.002 4,0.053 1±0.009 2和0.107 4±0.002 9。③肝癌细胞多倍体检测:肝癌原发灶组超5倍体比例为(25±6)%,而门静脉癌栓组是(58±9)%。各组间和各两组间差异均有统计学意义(均P<0.01)。[结论]门静脉癌栓中E-CD和CD44蛋白表达减少以及DNA超5倍体的增多是肝癌细胞向门静脉转移和门静脉癌栓向肝内转移的重要原因。[Objective] To explore expression of CD44 and E-CD changed in portal vein tumor thrombosis （PVTT） and analyze its role in intrahepatic metastasis. [Methods] Expressions of E-CD protein and CD44 protein in 33 cases of PVTT were detected by immunohistochemistry method. Their results were compared to primary tumor and paratumor tissue groups. Pentaploidy exceeding cells were counted under microscopy both in primary hepatic cancer and in PVTT. [Results] The average light-gaining index of expression of E-CD protein in PVTT group, primary tumor group and paratumor tissue group was 0. 0152±0. 0036,0. 052 7± 0.0063. respectively. CD44 protein was 0. 015 9±0.0024.0. 053 1±0. 009 2 and 0.1074±0.0029 respectively. The rates of pentaploidy exceeding cell in primary ma and in PVTT were （25±6）% and （58±9）% respectively. There was significantly difference among those three groups （P〈0. 01）, [Conclusion]The expression of E-CD protein and CD44 protein decreased in PVTT increased the possibility of tumor ecchysis and metastasis from PVTT.On the other hand,pentaploidy execeding cell in primary hepatocellular carcinoma directly invaded into portal vein to form TVTT.
