摘 要:目的探讨分泌性富含半胱氨酸的酸性蛋白(SPARC)在食管鳞癌(ESCC)增殖、浸润、转移中的作用和意义。方法用半定量逆转录聚合酶链反应(RTPCR)分析SPARC在24份ESCC组织和其相邻的正常黏膜组织的差异表达量及基因表达变化趋势。用酶联免疫吸附试验(ELISA)检测40名正常对照者和32例ESCC患者手术前后血浆中SPARC水平变化。结果经RTPCR分析SPARC在24份配对标本中的表达,其中19份ESCC组织中的表达明显高于其配对的正常黏膜组织,5份差异表达不明显;而在不同分化程度组织中的表达差异无统计学意义(P=0.663),淋巴结转移组与未转移组的差异有统计学意义(P<0.05)。正常人血浆SPARC水平[(644±124)ng/ml]与ESCC术前患者[(613±119)ng/ml]的差异无统计学意义(P=0.829);ESCC患者术后1周血浆SPARC水平[(245±45)ng/ml]明显低于术前(P<0.01),而19例ESCC术前患者血浆SPARC水平与术后3个月[(661±182)ng/ml]的差异亦无统计学意义(P=0.935);不同分化程度的ESCC患者血浆SPARC水平差异无统计学意义(P>0.05)。结论组织中SPARC水平与ESCC的发生与发展密切相关,有淋巴结转移时SPARC表达升高,血浆SPARC水平对ESCC诊治无指导意义。Objective To investigate the role of secreted protein, acidic and rich in cysteine (SPARC) in proliferation and metastasis of esophageal squarnous cell carcinoma (ESCC). Methods The differential gene expression of SPARC between esophageal squamous cell carcinoma and its corresponding normal esophageal mucosa from 24 patients was analyzed by semi-quantitative reverse transcription- polymerase chain reaction ( RT-PCR). Moreover, plasma samples from 32 patients with ESCC and 40 normal individuals were assayed for SPARC using a competitive ELISA. Results The gene expression of SPARC in ESCC by RT-PCR was higher than that in the corresponding normal esophageal mucosa. But it was not associated with the histological differentiation of ESCC. The mean plasma SPARC concentration in the pre-operation patients with ESCC [ (613 ± 119 )ng/ml ] was higher than that in post-operation ones [ (245 ± 45) ng/ml,P〈0.01], whereas as well as the controls [ (644 ± 124) ng/ml,P =0. 829]. No significant relationship between the levels of plasma SPARC and histological differentiation. The mean SPARC concentration in plasma three months after operation [ (661 ± 182) ng/ml] was not compared to those before operation [ (613 ± 119) ng/ml,P = 0. 935 ]. Conclusion These findings indicate that SPARC gene may be closely associated with ESCC and it is a tumor-associated gene. The up-regnlation of SPARC is marked During the metastasis of breast cancer.
