海洛因依赖者尿液中部分滥用物质检测分析

摘　　要：目的:通过海洛因依赖者尿液中部分非海洛因相关滥用物质及其代谢产物、烟草相关物质及代谢产物的检测分析,了解海洛因依赖者非海洛因相关滥用物质的滥用情况,为有关部门制定相应的防治药物滥用对策提供科学依据;了解海洛因依赖者的烟草滥用状况,为有关部门制定相关的控烟对策提供科学依据。方法:随机抽取海洛因依赖者30例,采集入院时的新鲜尿样,调pH值至5.0,用!-葡萄糖醛酸酶酶解。酶解后的样品分别调pH值为5.0、7.0、10.0,过SepC18小柱,分别用2ml甲醇、乙腈、二氯甲烷各冲洗一次,合并甲醇、乙腈、二氯甲烷冲洗液,用氮气流吹,浓缩至0.5ml左右,采用气相―质谱联用法(GC-MS)检测滥用物质及其他成份。结果:30例尿样中均检测出海洛因相关的滥用物质,检出非海洛因相关滥用物质15例,占50.0%。非海洛因相关滥用物质主要为巴比妥、苯丙胺类、安定等。检出烟草相关物质28例,占93.3%。结论:⑴海洛因依赖者存在着严重的物质多重复合滥用问题。⑵海洛因依赖者中吸烟者占有相当高的比例。Objective： To identify abused drugs except opiaties in the heroin addict＇s urine, for example barbiturates, amphetamine-type stimulants, benzodiazepines, nicotine, etc. Methods： The heroin addict＇s urine was adjusted to pH 5.0 with 1.0 mol·L^-1 HCI and hydrolysed with β-glucuronidase for 1 hour at 37℃. The pH of samples was adjusted to 5.0, 7.0, 10.0 separately and they were extracted by C18 Sep-Pak cartridges, washed with 2ml methanol, 2ml acetonitrile and 2ml dichloromethane. The organic extracts were dried with N2. The abused drugs except opiaties were identified by gas chromatography-mass spectrometry （GC-MS）. Results： 15 cases of abused drugs except opiaties were identified in 30 cases. The main abused drugs except opiaties were barbital （6 cases）, amphetamine-type stimulants （5 cases）, diazepam （3 cases）, etc. 28 cases of nicotine and cotinine were identified. Conclusion： The rate of multiple drug abuse was high among heroin addicts. The rate of cigarette smoking was high among them too.
