摘  要:目的探讨足月新生儿生理性黄疸对早期脑功能和听力的影响。方法选择120例足月生理性黄疸新生儿,按血清胆红素水平不同分为2组。在黄疸高峰期后1~2 d 内分别行新生儿行为神经测定(NBAN)、听力检查,并进行相关因素分析。结果血清胆红素水平高于171 μmol/L的生理性黄疸足月儿NBNA异常率、听力检查的未通过率明显升高;黄疸消退后,2组的NBNA、听力检查明显恢复。NBNA主要失分项目为反映视听定向力的行为能力3项;多因素分析显示,血清胆红素高峰值、胎龄、黄疽高峰时日龄、出生体重 4项因素对生理性黄疸新生儿NBNA的影响显著。结论新生儿血清胆红素水平高于 171μmol/L的生理性黄疸可出现一过性的轻微脑功能改变,是否出现远期神经系统后遗症需要进一步随访观察。Objective To investigate the early effect of physiological jaundice on brain function and to explore factors associated with brain function among term infants. Methods The study included 120 infants with physiological jaundice term infants. All the infants were underwent Neonatal Behavioral Neurological Assessment (NBNA) and hearing screening within one to two days following the jaundice peak. Results Infants with bilirubin level higher than 171 μmol/L had increased rates of abnormal NBNA scores and abnormal hearing screening results. However, NBNA scores and hearing screening results were improved with the jaundice fading away. In the NBNA examination, most of infants were subtracted scores in the behavior capability items which refleet the directional capability of sight and hearing. Multivariate analysis showed that peak bilirubin level, gestational age, age, and birth weight were associated with NBNA scores. Conclusion In the present study, the physiological jaundice group with bilirubin level higher than μmol/L had slightly transient brain function damage.
