摘 要:目的:探索人巨细胞病毒(HCMV)宫内感染产前诊断的最佳方法。方法:应用酶联免疫吸附试验(ELISA)、巢式PCR(N-PCR)和逆转录PCR(RT-PCR)对比分析武汉、上海和沈阳3地3908例孕妇HCMV感染状况及其对妊娠结局的影响。结果:孕妇HCMV IgM检出率为6.24%,与HCMV DNA和晚期mRNA的检测符合率分别为59.03%和56.49%。活动性感染孕妇的宫内传播率(41.58%)明显高于对照组(15.87%)。HCMV晚期mRNA阳性的宫内感染儿流产、早产发生率明显升高。采集绒毛、羊水和脐血进行产前诊断的安全性良好。结论:在应用ELISA法进行初筛的基础上,联合应用N-PCR和RT-PCR技术检测HCMV DNA和晚期mRNA表达,是产前诊断HCMV宫内感染安全而可靠的方法之一。Objective: To explore the best prenatal dagnostic Method for intrauterine human cytomegalovirus (HOMV)Infection.Methods: Enzyme-linked imrnunosorbent assay (ELISA) combined with nasted-polymerase chain reaction(N-PCR) and reverse transcription-PCR (RT-PCR) technique were used to study the state of HCMV infection and the pregnancy outcome in 3908 pregnant women in Wuhan,Shanghai,and Shenyang. Results:The positive rate of HCMV IgM antibody was 6.24% ,and the accordant rate of HCMV DNA and late mRNA with it was 59.03% and 56.49%, respectively. The intrauterine transmission rate of pregnant women with active HCMV infection (41.58%) was significantly higher than that of the control group (15.87%).The rate of abortion and preterm delivery increased significantly in the intrauterine infective fetus with positive HCMV late mRNA. The prenatal diagnosis collecting from chodonic villus,amniotic fluid and/or cord blood was safe. Conclusions:After screening of HCMV infection during pregnancy by EUSA, the detection of HCMV DNA and late mRNA by N-PCR as well as RT-PCR is a safe and reliable method for prenatal diagnosis of the intrauterine HCMV infection.
