摘　　要：目的探讨解剖学重建双侧唇裂的口轮匝肌功能,减少术后畸形。方法选择8个月至21岁的双侧唇裂患者,根据年龄和畸形程度,选用原长法或加长法。术中在鼻翼基底的下方,将异常附着于鼻孔、梨状孔边缘的鼻唇束切断,然后沿鼻翼外缘转向外上切断,止于鼻翼外侧脚及深部骨膜的鼻束,将裂隙两侧的口轮匝肌附着端向近中水平旋转、复位,并梳理为深浅两层,浅层梳理为鼻束、鼻唇束,对位缝合两侧同名口轮匝肌断端,恢复其正常的环形结构。结果本组患者术后切口均Ⅰ期愈合,口轮匝肌运动时上唇各部分协调一致,上唇及鼻静态、动态外形满意。结论双侧唇裂行口轮匝肌解剖学重建术,可获得满意的外形及功能效果。Objective To reconstruct the function of orbicular muscle of mouth in bilateral cheiloschisis patients and reduce the postoperative deformities. Methods The patients who suffered from the bilateral cleft lip from 8 months to 21 years old were performed the operation. The operative method was used by whether rect ilinear or prolate method according the patients＇ age and their deformed degree, under outsole of ala nasi we can cut off nasolabial muscle bundle which is abnormal cohere at edge of nostrils and aperture piriformis, then along outer margin of ala nasi to change direction to alinasal outside upper to cut off nasal muscle bundle which is stopped by alinasal lateral angle and periosteum in deep part. Toward mesiocclusion level to revolve and reset orbicularis oris of fissure two side, and make it separate to deep layer and superficial layer. To make nasal muscle bundle and nasolabial muscle bundle in superficial layer and then sut. Two side isonym orbicularis otis＇ broken ends which is contrapositive, and recover its normal ring shaped architecture. Results Every postoperative incision healed in one - stage, all parts of upper lip can coordinate when the orbicular muscles move. The upper lip and nasal contours on quiet and dynamic state were satisfactory. Conclusion The excellent lip contour and the functional effect could be obtained through the dissecting reconstruction of orbicular muscle of mouth in bilateral cheiloschisis.
