摘 要:目的:通过肝立体解剖断层、三维超声立体断层图和三维超声能量图三种方法,对肝血管的立体走行和空间位置关系进行分析研究,创建肝叶、肝段和段内血管的三维超声立体图像,为肝占位性病变的精确定位诊断提供解剖学基础。方法:①选用经甲醛液浸泡固定的尸体肝标本15例,按肝叶间裂和段间裂及其2cm平行线进行切割,对断层面上血管的立体走行及空间位置关系进行观察、记录并拍照;②以肝立体断层解剖图为基础,用三维超声立体断层成像法和三维超声能量成像法,采集正常人体肝叶、肝段和段内血管的三维超声立体构筑图像。结果:①获得了22幅肝三维立体断层解剖图,在不同层面显示了肝不同部位的血管立体走行和空间位置关系。②利用三维超声立体断层成像法和三维超声能量成像法采集了正常人体肝各肝段和段内血管的三维立体断层图,显示的血管走行和空间位置与三维立体断层解剖图一致。结论:肝血管三维立体断层解剖图可作为三维超声立体断层成像和三维超声能量成像的解剖学依据。Objective To investigate the imageological basis of precisely localized diagnosis for localization relationship of liver and create three-dimensional ultrasonographic stereoscopical imaging of blood vessel of liver lobar, liver segment and liver intersegment by three kinds of imageries of liver stereo sectional anatomy,three-dimensional ultrasonographic stereo sectional map(3D-SSM) and three-dimensional color Doppler power imaging(3D-CDP). Methods ①Fifteen liver specimen in vitro fixed with formaldehyde were resected according to liver interlobar fissures, intersegmental fissure and parallel line of 2 cm, and vessel distribution and stereo position were observed, recorded and photographed on each liver incision.② Based on liver stereoscopical sectional anatomy, both of 3D-SSM and 3D-CDP of normal blood vessel of liver lobar, liver segment and liver intersegment were obtained with two kinds of techniques of them and used into the localization diagnosis for liver tumor. Results ①It showed vascular stereo distribution and special position of different location on the different incision of 22 liver stereo sectional imaging. ②It is the same with the result of three-dimensional stereo sectional anatomy map that of 22 3D-SSM of normal vessel of liver segments and intersegments by using both of techniques. Conclusions Three-dimensional stereo sectional anatomy of liver blood vessel may be taken as anatomical evidence that 3D-SSM and 3D-CDP was reconstructed.
