原发性或隐源性癫痫儿童行为问题和自我意识评价及其影响因素

摘　　要：目的探讨原发性或隐源性癫痫儿童行为问题和自我意识现状,以及影响因素。方法应用Achenbach儿童行为量表(CBCL)和Piers-Harris儿童自我意识量表(PHSS)对68例原发性或隐源性癫痫儿童进行评定,并将结果与中国常模进行比较。同时应用回归分析对其影响因素进行分析。结果原发性或隐源性癫痫儿童的行为问题的异常检出率为44.12%,明显高于全国常模(P<0.01),且突出表现在多动(20.59%)、交往不良(14.71%)和强迫行为(14.71%)等因子。原发性或隐源性癫痫儿童自我意识总分及行为、智力与学校情况、躯体外貌与属性、幸福与满足等因子的分值明显低于全国常模(P<0.05)。对年龄、性别、受教育程度、病程、起病年龄和癫痫发作控制情况等多个变量多元逐步线性回归分析发现,癫痫发作控制与否是影响其精神行为问题的最显著的因素。而性别与起病年龄也是显著影响因素。结论原发性或隐源性癫痫儿童存在精神行为问题,且自我意识水平偏低。癫痫儿童的精神问题受着癫痫发作控制情况、起病年龄和性别的显著影响。Objective To study the behavioral problems and self-concept in children with idiopathic or cryptogenic epilepsy as well as the influencing factors. Methods A total of 68 children with idiopathic or cryptogenic epilepsy were assessed with child behavior checklist（CBCL） and Piers-Harris children＇s self-concept scale （PHSS）. The results were compared with the chinese norms and the regression analyze was conformed. Results The ratio of the children with abnormal behavior was 44. 12% and significantly higher than the chinese norms. And the scores of PHSS in children with idiopathic or cryptogenic epilepsy were lower than the chinese norms. Moreover, the status of seizure control was the significant factor influencing the behavioral problems. And the gender and age at onset of epilepsy were also the important factors. Conclusion The children with idiopathic or cryptogenic epilepsy have behavioral disorders and low self-concept. With the well control of seizure the children have the low frequency in behavioral and psychiatric disorders. The boys with idiopathic or cryptogenic epilepsy have higher risk of behavioral disorders than the girls. Moreover, with the earlier age at onset of epilepsy the children have the higher risk of behavioral and self-concept problems.
