摘  要:目的:探讨不同运动形式对冠心病患者心肺机能和运动能力的影响。方法:将44名老年冠心病患者随机分为单纯有氧练习组(A组)和有氧加力量练习组(AR组);按处方进行12周的康复锻炼。实验前、后改良Bruce递增负荷,采集HR、血压、VO2、VE、SV、CO、EF、FS,进行RPE及15RM力量测试。结果:康复后AR组安静时SV增加显著优于A组;Bruce递增负荷运动持续时间延长,同等运动强度下RPE降低,AR组由于肌肉力量的显著增长(P〈0.01),其提高幅度更为明显。结论:有氧加力量练习比单纯有氧练习能更明显地提高冠心病患者的心肺机能;力量练习强度主要以%RM值及RPE来控制,50%-60%15RM强度的力量练习不会加重冠心病患者心肌缺血现象,并可以提高其康复效果。The purpose of this study was to analyze the effect of different exercise style on the CHD patients' cardiopulmonary function and work capacity. 44 CHD patients were divided into 2 groups: pure aerobic training group (A) and aerobic combine with resistance training group (AR). All patients performed the graded exercise on the modified Bruce treadmill protocol. Before and after the exercise,we test the metabolism parameter, blood lactate, blood pressure, left ventricular parameter such as SV, CO, EF, FS, measured the RPE as well as their 15 repetition maximum (15RM) strength. The result showed that after rehabilitation, AR groups' SV was significantly higher than A group. The time they performed the Bruce protocol became longer,and the RPE at a given work load is decreased. AR group's muscle power was increased significantly (P〈0.01) so that their progress seems more obvious. It concludes that aerobic combined with resistance training was superior to pure aerobic training in increasing CHD patients' cardiopulmonary function and work capacity. 50-60% 15RM was a suitable intensity of resistance training;it couldn't aggravate myocardial ischemia but would increase the effects of rehabilitation indeed.
