西藏碘缺乏病国际合作项目阶段评估及消除碘缺乏病策略考察报告

摘　　要：目的评估西藏碘缺乏病国际合作项目取得的成绩和存在的不足,为调整今后综合干预项目的策略提供依据;考察西藏消除碘缺乏病工作,了解普及碘盐实现2010年目标的障碍,探讨解决的对策。方法和活动在拉萨、山南、林芝3个地区听取当地汇报、座谈和现场考察。结果西藏自治区政府重视碘缺乏病工作,出台了政策,但落实不到位;食盐加碘8年,2/3的居民还没有吃上碘盐。主要原因是碘盐运价高,销售网络覆盖低,市场监管无力。专家组认为澳援项目总体上是成功的,促进了西藏消除碘缺乏病的防治工作步伐,增强了政府重视和部门参与,提高了专业队伍水平。建议与国家策略保持一致,实行以食盐加碘为主、投服碘油丸为辅的综合防治策略;政府实行盐价补贴,切实解决西藏碘盐普及问题;盐业转变经营方式;推广典型经验,以点带面;开展多种形式的健康教育活动。Objective To evaluate the achievement and shortage existed in the implementation of IDD international cooperation program in Tibet in order to provide bases for strategy making and amendment; to review the efforts on IDD elimination in Tibet and understand the barriers for achieving the goal of 2010 as well as its solution. Methods and Activities Field visit was conducted at autonomous region and district level including Lasa, Shannan and Linzhi; interviewing and meeting were held with target population; work report was reviewed and discussed. Results The Tibet government paid attention on IDD elimination and issued some relevant regulations, but it was not well carried out; USI has been implemented over 8 years, but two - part residents still could not take iodized salt. The main reasons are the high transportation fee, the low coverage rate of iodized salt selling network and weakness on inspecting and management of the salt market. The experts group thinks that the AusAid support program is successful in general. The program accelerated the progress of IDD elimination in Tibet; it facilitated the government recogni- tion and multi - sector involvement; it improved the technical level of the professional team. Suggestions The strategy should be adjusted according to the country overall program strategy that is USI in general and supplemented by iodized oil capsule for special group; gaining the government additional supplement to resolve the price problem of iodized salt; changing the selling model of salt sectors according to the local practice; summarizing and disseminating the successful experiences; carrying out various and attractive health education activities.
