甲状腺良恶性病变组织中CTGF的表达及临床意义

摘　　要：目的检测结缔组织生长因子(CTGF)在甲状腺良恶性病变组织中表达水平及其与临床参数的关系。方法应用免疫组化技术检测72例甲状腺癌(A组)、45例甲状腺瘤(B组)、71例单纯性甲状腺肿(C组)组织中CTGF的表达水平,并分析其与临床参数的关系。结果A组有58.3%(42/72)CTGF表达上调,B、C组分别有60%(27/45)和87.3%(62/71)CTGF表达下调。CTGF表达水平与甲状腺癌相关的临床参数包括患者性别、淋巴结转移、肿瘤体积、病理分级和促甲状腺激素刺激激素(TSH)。与甲状腺瘤相关的临床参数为患者家族史、肿瘤大小和游离甲状腺素(FT4),与单纯性甲状腺肿的相关临床参数为患者性别、FT4和TSH。结论CTGF在甲状腺癌组织的高表达可能是肿瘤发生过程中的一个重要因素,其表达的蛋白产物可作为临床上甲状腺癌早期诊断和治疗的一个重要指标。Objective To investigate connective tissue growth factor （CTGF） protein expression levels in thyroid tissues and their relation to clinical and pathological features in thyroid cancer. Methods CTGF protein expression levels were quantified in 72 pairs of thyroid papillary carcinoma（A） ,45 pairs of thyroid adenoma（B） and 71 pairs of simple goitre （C） and their matched thyroid tissues by using immunohistochemistry staining method. Their correlation with clinical and pathological features was analyzed. Results CTGF protein expression was up-regulated in Group A to 58.3% （42/72）, down-regulated in Group B and C to 60% （27/45） and 87.3% （ 62/71 ） respectively. Univarlate analysis proved that the protein expression of CTGF in thyroid papillary carcinoma was strongly associated with gender, lymph node metastasis, tumor size, pathological grade and TSH level. The CTGF protein expression level in thyroid adenoma was strongly associated with some clinical features, including family history, tumor size, and the level of FT4. The expression of CTGF protein in simple goitre also had a significant association with gender, FT4,TSH. Condusions Up-regulation of CTGF in thyroid cancer may play an important role in the progression of thyroid cancer, and the protein of CTGF might serve as a valuable target for early diagnosis and clinical therapeusis.
