摘 要:目的:观察病变侧亚低温对大鼠局灶性脑缺血再灌注损伤后tPA和PAI-1表达的影响,在缺血再灌注不同时程和不同部位的表达及意义。方法:用改良线栓法制作大鼠大脑中动脉阻塞(MCAO)局灶脑缺血再灌注模型,随机分为假手术组、常温缺血再灌注组和亚低温缺血再灌注组,于缺血2小时分别再灌注4小时、24小时、72小时、7天、14天后处死,其中亚低温组于缺血后30分钟实施病灶侧脑亚低温并持续4小时。处死前进行神经功能缺陷评分;HE染色观察病理形态学改变;免疫组织化学法检测tPA和PAI-1的表达,并进行定性和定量分析。结果:亚低温组大鼠神经元损伤明显减轻,神经功能缺陷评分明显低于常温组(P<0.05),脑组织tPA表达水平较常温组显著减少(P<0.05),PAI-1表达显著增高(P<0.05)。结论:亚低温能显著抑制tPA在脑缺血再灌注损伤后的表达,增加PAI-1表达,从而有利于抵御脑缺血再灌注造成的脑损伤。Objective: To observe the effect of mild hypothermia on the immunoreactivity of tissue plasminogen activator (tPA) and plasminogen activator inhabitor-1 (PAI-1) after focal cerebral ischemia and reperfusion , and to investigate the change of them expression in different ischemic areas and at different time. In order to study their relationship and value. Methods: Reversi- ble middle cerebral artery occlusion was performed with modified Koizumi's model . Rats were randomly divided into sham-operated group, normal ischemic/reperfusion group and mild hypothermia ischemic/reperfusion group. Normothermia ischemic group and hypothermia ischemia group were respectively reperfused for 4h,24h,74h, 7d and 14d after ischemia for 2h. Brain mild hypothermia was achieved 80 minutes after ischemia and maintained to 4h. The neurological deficit scores were determined before killing , pathohistological changes of injured neurons were demonstrated by H-E staining and microscopy , and the expression of tPA and PAI-lwas assayed by immunohistochemistry and computerized image analysis system. Results: The expression of tPA in mild hypothermia group significantly decreased compared with nomothermia group (P〈0. 05) ,the expression of PAI-1. significantly increased (P〈0.05). Conclusion: Mild hypothermia can reduce expression of tPA and increase expression of PAI-1. In a focal model of transient cerebral ischemia reperfusion in rats. mild hypothermia treatment also resists the brain injured caused by transient cerebral isehemic reperfusion.
