偏头痛患者外感受抑制实验

摘  要:目的探讨三叉神经颞肌、咬肌外感受抑制(ES)检测偏头痛的应用价值。方法利用肌电图刺激眶下神经对20例偏头痛患者及30名健康对照者进行ES试验,检测颞肌、咬肌ES的潜伏期和时限。结果偏头痛患者第二外感受抑制期(ES2)潜伏期均数值为49.6~55.3ms,时限为26.6~31.3ms,健康对照组分别为48.8~53.0ms和29.5~33.9ms,两组间比较差异无统计学意义;偏头痛组中70%(14/20)患者存在一些特征性的较高程度抑制的波形。结论具有特征性的较高程度抑制的波形提示偏头痛存在外感受抑制环路中间神经元的过度活动;ES实验对了解偏头痛的电生理变化可提供一定帮助。Objective To investigate the application value of the trigeminal exteroceptive suppression (ES) of temporalis and masseter muscles to examine migraine sufferers. Methods Twenty patients with migraine and thirty normal volunteers were examined with electric stimulated infraorbital region utilizing electromyogram, to measure the latency and duration of ES of temporalis and masseter muscles. Results The latency of the second exteroceptive suppression period (ES2) was 49. 6-55.3 ms (mean) and the duration of ES2 was 26.6-31.3 ms in migraine group; the latency of ES2 was 48.8-53.0 ms and the duration of ES2 was 29.5-33.9 ms in control group. Although there was no significant difference of the numerical date between the two groups, some distinctive higher degree suppressive curves were evoked in 70% (14/20) patients with migraine. Conclusions Distinctive higher degree suppressive curves in migraine group suggested overactivity of the interneurons of this reflex loop, which showed the examinations of ES might be helpful to understand the electrophysiological change of migraine.
