摘 要:目的探讨人冠状动脉粥样斑块中白细胞介素6和18及肿瘤坏死因子α表达水平与斑块稳定性的关系,以及白细胞介素6和18及肿瘤坏死因子α之间的相关性。方法选取104例冠状动脉组织蜡块标本,在光学显微镜下观察,并结合SmartScape图像分析系统对冠状动脉粥样斑块面积、脂核面积和纤维帽厚度进行测量分析,将其分为正常血管组、稳定斑块组和不稳定斑块组。采用免疫组织化学技术检测冠状动脉粥样硬化斑块中白细胞介素6和18及肿瘤坏死因子α的表达水平。结果稳定斑块组与不稳定斑块组斑块面积差异无显著性(P>0.05),而斑块脂核面积和纤维帽厚度在两组间差异有显著性(P<0.01)。正常血管中白细胞介素6和18无表达,肿瘤坏死因子α无表达或极少量表达;在不稳定斑块组白细胞介素6和18及肿瘤坏死因子α表达水平明显高于稳定斑块组(P<0.01),尤其是在纤维帽的肩部和脂质核心。白细胞介素6和18及肿瘤坏死因子α表达具有相关性,且各因子间呈正相关关系。结论白细胞介素6和18及肿瘤坏死因子α表达水平可反映冠状动脉粥样硬化斑块的稳定性,并且它们之间的表达具有相关性。检测这些因子可帮助临床医生预测冠心病危险事件的发生。Aim To probe the relationship between plaque stability and expression levels of intedeukin-6 ( IL-6), interleukin-18 (IL-18) and tumor necrosis factor-α (TNF-α) in human coronary atherosclerosis. And to investigate the correlation of IL6, IL-18 and TNF-α. Methods One hundred and four autopsy samples of coronary paraffin sections were selected and divided into unstable plaque group, stable plaque group and normal coronary group by means of SmartScape biology miscroscope analysis system. Plaque area, lipid core area and fibrous cap thickness in the three groups were measured and analyzed. The expression levels of IL-6, IL-18 and TNF-α in plaque of human coronary atheroscleresis were measured by immunohistochemical technique. Results There was no statistically and significant difference between the unstable plaque group and the stable plaque group in the area of coronary atheroscleresis plaque ( P 〉 0.05), but the lipid core area and fibrous cap thickness were statistically and significanly different between them ( P 〈 0.05). IL-6, IL-18 were not expressed, and TNF-α was httle or not expressed in the nomal coronary group. The expression levels of IL-6, IL-18 and TNF-α were higher in the unstable plaque group than in the stable plaque group, especially in the shoulder of fibrous cap and lipid core ( P 〈 0.01 ). There was positive correlation among the expression of IL-6, IL-18 and TNF-α. Conchlsions The expression levels of IL-6, IL-18 and TNF-α in plague of human coronary atherosclerosis was not only related to plaque stability but also correlated among one another.
