童年期虐待与青少年自我伤害行为关系

摘　　要：目的探讨童年期不良经历与青少年自我伤害行为的关系。方法在安徽省4个县各方便抽取4个中学。每所中学分别在初一至高三每个年级随机整群抽取1～2个班级，按照自愿参加的原则对学生进行匿名问卷调查。共获得有效问卷10894份。结果童年期反复重度躯体虐待报告率为8．5％；反复中度躯体虐待报告率为23．6％；反复情感虐待的报告率为20．4％。青少年总体自伤报告率为22．3％，自伤的报告率与3种童年期反复虐待的经历呈相关趋势。童年期3种虐待经历是青少年自我伤害行为发生的危险因素。结论不同类别和程度的童年期反复躯体虐待和反复情感虐待经历对青少年自我伤害行为产生负性影响。Objective To examine the relationship between childhood abuse and adolescents＇ self- injurious behaviors. Methods The investigation was administered to 10 894 middle ：school students from 16 schools in 4 counties of Anhui province. All participants completed an anonymous questionnaire with information of demography, adverse childhood experiences and self- injurious behaviors. Results The incidence of repeated severe childhood physical abuse was 8.5 96, in which the lowest rate was 0.7 96 （ head being pressed under water） and the highest was 5.9 96 （being kicked）. There were 23.6 96 of respondents exposed to repeated moderate childhood physical abuse, in which the lowest rate was 1.5 96 of being forced to eating their disliked food and the highest was 13.7 96 of being spanked buttocks with hand or sticks. The incidence of repeated childhood emotional abuse was 20.4 96, in which the lowest rate was 1.9 96 （being threatened by speaking of evil spirits） and the highest was 14.8 96 （being shouted）. The rate of self - injurious behaviors was 22.3 96. Dose - response trend existed between the number of items of repeated serious childhood physical abuse, moderate childhood physical abuse, childhood emotional abuse and the rates of general self - injurious behaviors and 6 kinds of self - injurious behaviors. Unconditional multivariate logistic regression results showed that the three forms of repeated childhood maltreatment were the risk factors for the adolescents＇ self- injurious behaviors. Conclusion Childhood abuses had negative influence on self - injurious behaviors in adolescents.
