食管癌介入性药物灌注治疗的解剖学观测

摘　　要：目的为临床上利用胸段食管动脉介入性治疗食管癌提供解剖学依据。方法采用成人尸体标本60具(男47,女13),解剖、观察胸段食管动脉的起点、分支与吻合;测量外径、夹角和可游离长度。结果114支胸上段食管供血动脉来源于支气管动脉,占该段食管供血动脉总支数的68.67%;66支胸下段食管供血动脉来源于支气管动脉,占该段食管供血动脉总支数的42.31%。胸下段食管动脉器官外分支可分为纵行多分支型和横行少分支型。食管胸段的供血动脉在器官外的相互吻合较少,器官表面的吻合较多。结论在选择性靶动脉造影时,应细心观察吻合支;只有寻找肿瘤全部供血血管并灌注后,胸段食管癌介入性药物灌注才能获得最佳效果。Objective To provide anatomical data about thoracic esophageal arteries for the intervention therapy of esophageal cancer. Methods The origin, the branches length, angles and the adjacent relations of thoracic esophageal adult cadavers （males 47, females 13）. Results 114 branc and anastomosis, the origin diameter, the dissociated arteries were dissected and measured in 60 Chinese hes of esophageal arteries supplying blood to esophagus originated from bronchial artery, accounting for 68.67 % of total esophageal artery in superior thoracic segment; 66 branches of esophageal arteries supplied blood to esophagus originated from bronchial artery, accounting for 42.31% of total esophageal artery in inferior thoracic segment. There were two types of artery branches on inferior thoracic esophagus, more branched vertical arteries and little branched transverse arteries. There were more atrial anastomosis on the surface of esophagus than those outside of esophagus. Conclusion While performing the selective target artery angiography, anastomosis branches should be carefully observed. The best results didn＇ t get for interventional treatment, until all the arteries supplying blood to esophageal cancer were found and perfused with drugs.
