从“胆瘅”病机看肝胆在神志方面的作用

摘　　要："胆瘅"是《素问.奇病论》明文记载的一种直接由神志活动引起的疾病,病位在"奇恒之府"——胆。对"胆瘅"病机和《素问.痿论》"筋痿"病机的比较研究证明,"胆瘅"一病,其病位虽然在胆,其病因却在肝。"胆瘅"的病机分析表明,"谋虑"和"决断"不是两种独立的神志活动,而是同一神志活动的过程和结果。因此,《素问.灵台秘典论》说肝主谋虑、胆主决断,不是指两者在调控神志方面的功能相互独立,而是肝胆共同完成神志调控作用。注家对"胆瘅"病机的解释多有牵强含混,重要原因即把"谋虑"和"决断"误当作两种独立的神志活动,从而割裂了肝和胆的这一密切联系。Gallbladder pure heat is a disease recorded in the ＂Yellow Emperor＇s Inner Canon＂ which originates from mental activity and focus on one of the extraordinary organ - - gallbladder. Comparative study between the pathological mechanism of gallbladder pure heat and sinew goiter shows that the cause of gallbladder pure heat is the pathological change of liver. ＂The making of strategies＂ and ＂decision＂ are not individual mental activities, but the process and ending of one mental activity. ＂Liver governs the making of strategies＂ and ＂gallbladder governs decision＂ imply not the division in mental management of liver and gallbladder, but the integration of mental function of liver and gallbladder, which were totally confused in many kinds of explanations of the disease.
