摘 要:背景与目的:Fascin蛋白是一种结构独特的细胞骨架蛋白,能促使活性细胞胞膜突起并增加细胞运动性。目前已经观察到Fascin在人类食管、胆管、乳腺、结肠、卵巢、胰腺和胃等上皮组织起源的肿瘤中有表达,其表达水平和部分肿瘤的预后有关。但Fascin与口腔鳞癌的研究较少。本研究应用组织芯片技术研究Fascin和Ki67在口腔鳞癌中的表达情况及其临床意义。方法:回顾性分析1991-2005年在我科诊治的84例口腔鳞癌患者,并将患者手术标本的病理蜡块制作组织芯片。应用免疫组化法检测Fascin和Ki67在口腔鳞癌组织中的表达情况。收集相关临床病理资料进行统计分析。研究淋巴结转移、T分期(T1与T2~4)、肿瘤分期(Ⅰ/Ⅱ期与Ⅲ/Ⅳ)、分化、肌肉侵犯(54例舌鳞癌)与Fascin的相关性。结果:Fascin高表达与淋巴结转移、较高T分期、较高肿瘤分期、较差分化相关;在54例舌鳞癌中与肌肉侵犯相关(P=0.002)。Fascin和Ki67表达呈正相关,两个指标中高表达与低表达患者的生存率间差异有显著性(P=0.034)。结论:Fascin在口腔鳞癌的进展中可能起有重要作用,可作为口腔鳞癌的肿瘤标志物,并与患者预后有关。Background and purpose: Fascin, an actin bundling protein, induces membrane protrusions and increased cell motility in various transformed cells.Overexpression of Fascin frequently occurs in various kinds of malignant tumors and is implicated in tumor progression, but the role of Fascin in oral squamous cell carcinoma (OSCC) is still unknown.The aim of this study was to explore the expression of Fascin and Ki67 in OSCC by tissue array analysis. Methods: Paraffin sections using tissue microarrays of 84 patients with OSCC were investigated immunohistochemically, to determine the level of expression of Fascin and Ki-67 labelling index in tumor specimens. The role of Fascin in OSCC was evaluated by correlation with clinical-pathological parameters. Results: Overexpression of Fascin was significantly associated with regional lymph node metastases (P=-0.044) ,advanced T stage (T1 vs T2-4, P=0.046) advanced tumor stage ( I/Ⅱ vs Ⅲ/Ⅳ, P=0.049), histologic grade (well vs moderately/ poorly, P=-0.018) and Ki67 (P=0.034). In 54 patients with tongue carcinoma, overexpression of Fascin was significantly associated with muscle invasion(P=0.002). The expression of Ki-67 was higher(P=0.017) in tumors with poorer grade of histologic differentiation. Fascin and Ki67 overexpression were all significantly associated with a poor prognosis (P〈0.001). Conclusion: Expression of Fascin protein may play an important role in progression of OSCC. Overexpression of Fascin contributes to a more aggressive clinical course and suggests that Fascin has the potential of being a new molecular target for therapeutic intervention. Fascin and Ki-67 overexpression were positively correlated.
