恶性胸腔积液血免疫指标与胸水癌胚抗原及染色体的关系

摘　　要：目的:研究恶性胸腔积液患者血免疫指标与胸水癌胚抗原(CEA)及染色体之间的关系。方法:回顾73例恶性胸腔积液患者的临床资料,分析患者的血免疫指标CD3+T、CD4+T、CD8+T、CD4+/CD8+、NK细胞及B细胞与胸水CEA、染色体间的关系。结果:(1)CEA(+)组中,CD4+T、CD4+/CD8+及NK细胞明显降低(P<0.01),CD8+T明显增高(P<0.01)。CEA与CD4+T、CD8+T、CD4+/CD8+及NK细胞水平密切相关(P<0.01)。(2)染色体畸变(+)组中,CD4+T、CD4+/CD8+及NK细胞明显降低(P<0.01),CD8+T及CEA明显增高(P<0.01)。结论:恶性胸腔积液患者存在不同程度的细胞免疫状态紊乱,且与胸水CEA及染色体畸变密切相关。Objective To explore the relationship of blood immunologic factors and pleural fluid eareinoembryonie antigen （CEA） and chromosome in patients with malignant hydrothorax. Methods The clinical data of 73 patients with malignant hydrothorax were analyzed retrospectively, and the relationship of blood immunologic factors （CD3＋T cells, CD4＋T cells, CD8＋T cells, CD4＋/CD8＋, NK cells and B cells） and CEA and chromosome were analyzed. Results The levels of CD4＋ cells, CD4＋/CD8＋, and NK cells were significantly decreased in CEA＋ group and chromosome aberration＋ group （P 〈 0.01 ）, CD8＋ T cells in CEA＋ group and CD8＋T cells and CEA in aberration＋ group were significantly increased （P 〈 0.01）. CEA was significantly related to the levels of CD4＋T cells, CD8＋T cells, and NK cells （P 〈 0.01）. Conclusion There exists cellular immunologic disorders in patients with malignant hydrothorax, and it is closely related to the CEA level and chromosome aberration in pleural fluid.
