摘 要:目的:对比两种取材方法进行先天性人巨细胞病毒(HCMV)感染产前诊断的准确率。方法:选取外周血HC-MV-IgM阳性的孕妇,联合运用显微操作技术、引物延伸预扩增法(Primed extension preamplification,PEP)及聚合酶链反应(PCR)分别检测羊水和孕妇外周血中胎儿有核红细胞(FNRBC)的HCMV基因,将检测结果与胎儿实际感染情况对比。结果:运用羊水产前诊断HCMV感染的检出率为57.14%(4/7),准确率为65.38%(17/26);运用FNRBC诊断胎儿HCMV宫内感染的检出率为83.33%(15/18),准确率为92.86%(39/42)。结论:通过孕妇外周血中胎儿有核红细胞产前诊断巨细胞病毒感染优于传统方法-羊水,可能为胎儿的取舍提供更充分的依据。Objective: To contrast the accuracy of two sampling method used to prenatal diagnose congenital HCMV infective. Methods: Micromanipulation techniques combined with primed extension preamplification (PEP) and polymerase chain reaction (PCR) were used to detect HCMV - DNA in amniotic fluid and single FNRBC respectively, in pregnant women whose HCMV - IgM was positive in the blood. Contrast the result with the fetus'real infection situation. Results: The detection rate and accuracy of detecting HCMV gene in amniotic fluid were 57.14% (4/7) and 65.38% (17/26) respectively; In FNRBC the detection rate and accuracy were 83.33% (15/18) and 92. 86% ( 39/42 ) respectively. Conclusion : The prenatal diagnosis of congenital HCMV infection by FNRBC in maternal blood is better than by amniotic fluid, and can provide more sufficient evidences for the decision of the acceptance or rejection of the fetus with congenital infection.
