摘 要:目的评价多层螺旋CT(MSCT)后处理技术在肺癌术前分期中的应用价值。方法选取212例经MSCT检查并经病理证实的肺癌患者,采用国际最新TNM分期标准进行诊断,利用多平面重组(MPR)、最大密度投影(MIP)、表面遮盖显示(SSD)、容积再现(VR)技术对所有CT扫描资料进行图像后处理,结合横断面图像评价其在肺癌前分期的作用,并与横断面图像的术前分期进行对照。结果对T分期,T2~T。轴位图像诊断符合率分别为77.3%、60.6%、60.7%、60.3%,轴位图像结合后处理图像诊断符合率分别为86.4%、90.9%、89.3%、87.0%;对N分期,N0~N3横断面图像诊断符合率分别为84.6%、68.1%、63.6%、64.2%,轴位图像结合后处理图像诊断符合率分别为92.3%、90.9%、90.9%、85.7%。除T1、L.外,T1~T3及N1~N3后处理图像和横断面图像诊断符合率比较差异均有统计学意义(P〈O.05)。而后处理图像对远处转移也可以提供一定帮助。结论螺旋CT后处理技术能提高肺癌TNM分期的准确性,能有效增加和帮助提高横断面CT的诊断信息。Objective To evaluate the role of spiral CT imaging post-processing techniques in preoperative TNM staging of lung cancer. Methods 212 patients with pathologically confirmed lung cancer underwent spiral CT. TNM staging method oflung cancer was taken as criterion of diagnosis. The origifml images were reconstructed by using different post-processing techniques such as MPR, MIP, SSD, VR. TNM staging by using post-processing images was analysed and compared with that of axial CT images. Resuits The accurate rates in T1 ,T2 ,Ta and T4 staging were 77.3% ,60.6 %, 60.7 % and 60.3 % respectively with axial CT images, and 86.4 %, 90.9 %, 89.3 % and 87.0 % respectively with postprocessing images. While the accurate rates in N0, N1, N2 and N3 were 84.6% ,68.1% ,63. 6% and 64. 2%respectively with axial CT images, and 92.3%, 90. 9%, 90. 9%and 85.7%respectively with post-pracessing images. There were significant differences in T1 , T2 and T3, N1 , N2 and N3 staging between axial images and post-processing images. For the metastases, the post-processing images might be helpful to the TNM staging of lung cancer. Conclusion Post-processing techniques of spiral CT can improve the accurate rate of TNM staging for lung cancer.
