发病年龄对重性抑郁障碍患者认知功能的影响研究

摘　　要：目的比较不同发病年龄重性抑郁症患者的认知功能状况。方法采用韦氏成人智力量表、韦氏记忆量表、威斯康星卡片分类测验分别评定123例首发抑郁症患者的认知得分并比较不同年龄间的统计学差异。结果言语智商、总智商、WCST分类数和WCST概念化百分数的差异有统计学显著性,其中言语智商的LSD事后检验显示:18～34岁组显著高于35～49岁组(P=0.000)、50岁以上组(P=0.019);总智商的LSD事后检验显示:18～34岁组显著高于35～49岁组(P=0.001)、50岁以上组(P=0.005)。WSCT分类数的LSD事后检验显示:18～34岁组显著高于35～49岁组(P=0.047)、50岁以上组(P=0.015);WSCT概念化百分数的LSD事后检验显示:18～34岁组显著高于50岁以上组(P=0.012)。结论不同发病年龄的抑郁症患者的认知功能不同,他们可能具有不同的神经心理病理基础。Objective This study aimed to compare cognitive function in patients with serious depression at different ages of on- set. Method： A total of 123 serious depressed patients were involved. The eognhive function was measured with Wechsler Adult Intelligence Scale（WAIS） ,Wechsler Memory Scale（WMS） and Wisconsin Card Sorting Test（WCST）. Results There was signif- icant difference in VIQ, IQ, WSCT classification numbers and WSCT conceptualization percentage. The LSD of VIQ indicated： those of 18 to 34 years old people were higher than those of 35 to 49 years old people and those of more than 50 years old people. The LSD of IQ indicated： those of 18 to 34 years old people were higher than those of 35 to 49 years old people and those of more than 50 years old people. The LSD of WSCT classification numbers indicated： those of 18 to 34 years old people were higher than those of 35 to 49 years old people and those of more than 50 years old people＇ The LSD of WSCT conceptualization percentage indi- cated： those of 18 to 34 years old people were higher than those of more than 50 years old people. Conclusion Cognitive functions are found different in patients with depression at different ages of onset. Patient with different onset may be with different neuro- logical, psychological and pathological basis.
