清除幽门螺杆菌过程中血清特异性IgG、IgA、IgM的变化

摘　　要：研究清除HP过程中血清抗体滴度的变化。方法51例pU伴Hp感染者，用三联疗法清除Hp，用ELISA法前瞻性定量检测血清特异性IgG、IgA、IgM的变化。结果治疗后9周，23例Hp转阴者，IgG、lgA、lgM平均滴度比治疗前分别下降14·2％、15．1％、7．2％。20周时分别下降32．5％、34．0％、7．0％。治疗前后IgG、IgA、滴度有显著性差异（t—3·83、3·56，P＜0·01）。治疗无效者16例，三类抗体呈上升趋势，其中9周时ItA、ItM滴度与治疗前相比有显著性差异（t=2．50、2．84，0．05＞P＞0．01）；12例9周转阴，而20周又阳性者，三类抗体呈先下降后升高趋势，尤以IgA、变化较为显著（F=3．46，0．05＞P＞0．01）。结论ELISA法定量检测IgG、IgA滴度的改变可以评价药物清除Hp的效果。Aim The change of antibody titres in serum was investigated after eradication of Hp Method Infection of HP from 51 pu patients with Hp infection were eradicated by tripe therapy and the change of spe-cific lgG.IgA.IgM to Hp in serum was measured prospectively by ELISA method. Resulte In 23 Hp-negative patients after treatment, IgG. IgA. IgM titres were decreased to 14. 2%' 15. I %' 7. 2% at 9 weeks and 32. 5%'34. 0% .7. 0% at 20 weeks respectively. The significant difference of lgG and IgA was observed before and after treatment (t= 3. 83 .3. 56, P<0. 01 ). 16 patients after treatment were ineffective and their titres of 3 --class Igwere raised slightly at 9 weeks to 20 weeks. The raise of lgA.lgM titres was significantly different between pre-treatment and 9 weeks after treatment (t= 2. 5 .2. 84,0. 05>P>0.01 ). In 12 cases the patients were negative at 9 weeks and then positive at 20 weeks for HP,the titres of 3--class Ig were first decreased and then increased.The change of IgA titres was more significan then other ig (F=3. 46,0. 05>P>0. 01 ). Conclusion The change of IgG.lgA titres measured quantitatively by ELISA may be used to evaluate the effects to eradicate HP.
