角膜全结膜瓣遮盖术式探讨

摘　　要：报告眼球萎缩及先天性小眼球１０例１０只眼行角膜上皮去除全结膜瓣遮盖术，９例９只眼均获满意效果，结膜瓣一期愈合，眼窝饱满，义眼活动好。１例术后４天结膜瓣裂开，７天退除至角膜缘。部分眼球萎缩及先天性小眼球患者可保留眼球，行角膜全结膜瓣遮盖术，安装义眼，改善外观。本文讨论了角膜全结膜瓣遮盖手术的关键步骤，手术的适应证。To explore the effectiveness and feasibility of entire conjunctival flap covering cornea operation,ten eyes of ten cases with bulbus atrophy and congenital microphthalmos were treated with entire conjunctival flap covering cornea operation.Outcomes were satisfying in nine eyes of nine cases,whose conjunctival flaps healed up well,eye sockets were plump,and prosthetic eye mobility and cosmetic appearance were excellent.Conjunctival flap of one case split at four days after operation and shrinked back to the limbus at seven days,and corneal epithelium was proliferative and repaired.The entire conjunctival flap covering cornea operation could treat most cases of ophthalmatrophia or microphthalmos and removing all of the corneal epithelium,especially in the limbus was the crux of success.Surgical methods,selection of the surgical technique and technical principal points were also discussed.
