摘  要:目的通过检测HPV16E6基因突变在新疆维吾尔族和汉族妇女宫颈癌组织中的分布规律,以探讨该突变与宫颈癌发生的关系。方法取汉族宫颈炎(含石蜡包埋组织及宫颈刷液基标本)125例,维吾尔族宫颈炎(含石蜡包埋组织及宫颈刷液基标本)124例;汉族石蜡包埋宫颈癌35例,维吾尔族石蜡包埋宫颈癌共109例。用以上HPV16阳性DNA模板PCR扩增HPV16E6全长基因,PCR产物直接测序,分析新疆女宫颈癌组织HPV16E6基因的突变。结果PCR检测结果显示汉族族宫颈炎组织中HPV16E6阳性率为35.71%(15/42);维吾尔族宫颈炎组织中HPV16E6阳性率为30.46%(14/46);汉族族宫颈癌组织中HPV16E6阳性率为33.33%(2/6),维吾尔族宫颈癌组织中HPV16E6阳性率为22.22%(12/54);宫颈炎与宫颈癌中HPV16E6阳性表达无统计学意义(P>0.05),对21份(上皮内低度病变1例,原位癌1例,低分化宫颈癌4例,中分化宫颈癌4例,高分化1例,轻度炎症4例,中度炎症3例,重度炎症1例,正常宫颈2例)HPV16E6扩增片段的双向测序,其中5例成功测序得到一级结构,并且序列分析表明,2例(维吾尔族宫颈癌低分化1例,中分化1例)其E6基因与原型相同,3例(汉族轻度炎症1例,汉族宫颈癌中分化1例,上皮内低度病变1例)其发生了L83V突变。结论中国新疆HPV16E6基因发生变异,其原型和变异型在该地区汉族、维吾尔族妇女宫颈癌患者组织中的分布可能与该地区宫颈癌高发存在一定的联系。Purpose The studies aimed at sequence analysis of HPV16E6 gene in this region indicated high frequency of mutation in HPV16E6 gene.Thus,this study was to detect the mutations of HPV16E6 gene and its distributions in cervical cancer tissues from the women in Xinjiang to further explore the possible association of mutations of this particular gene with high incidence of cervical cancer in this region.Methodes This study included 124 cases of cervicitis in Han women and 124 cases Uighur women,with both paraffin-embedded tissues and LCT samples,and also 35 cases of cervical carcinoma in Han women and 109 cases in Uihgur women.The mutation of HPV16 gene in thosr Xinjiang women with cervical carcinoma was studied by sequencing the whole HPV16 gene in the tissue specimen positive for HPV16 after PCR amplification,using DNA samples as templates extracted from the cervical cancer tissues.Results The PCR amplification studies detected HPV16E6 in the cervicitis tissues with positive rate of 35.71%(15/42) and 30.46 %(14/46) in Han and Uighur women,respectively;whereas in positive detection rate of HPV16E6 of in cervical cancer tissues from Han and Uighur women were 33.33%(2/6) and 22.22%(12/54),respectively;The difference in positive rate of HPV16E6 was not significant when compared between cervicitis and cervical cancer population in both ethnic groups(P〉0.05).Bi-directional sequencing analysis of HPV16E6 of cervical tissue samples from 21 subjects,including 1 case of low-grade squamous intraepithelial lesion(LSIL),1 carcinoma in situ,4 poorly-differentiated cervical carcinomas,4 moderately-differentiated cervical carcinomas,one with well-differentiated cervical carcinoma,4 mild inflammation,3 moderate inflammation,1 severe inflammation and 2 normal cervical tissues.HPV16E6 gene was successfully amplified and sequenced in cervical tissue samples from five of these 21 subjects.Primary sequence analysis of HPV16E6 in the samples from these five individuals indicated that only two subjects(Uighur)
