颊脂垫移植的临床应用及组织学研究

摘　　要：目的:为了解带蒂颊脂垫组织瓣的应用解剖及颊脂垫移植修复口腔内恶性肿瘤切除软组织缺损的愈合过程。方法：在尸体和活体观察的基础上总结颊脂垫组织瓣的解剖特点，并用于修复口腔颌面部恶性肿瘤切除后软组织缺损112例，还对颊脂垫移植后的愈合过程进行了组织学观察。结果：口腔内颊脂垫移植后6～8周完全上皮化，随访3个月至8年6个月，效果良好。结论：该组织瓣解剖恒定，易于切取，供区与受区邻近．可在同一切口内完成手术，供区隐蔽，不影响外观及功能，为修复口腔内软组织缺损提供了一种新的方法。Objective To introduce the applied anatomy and clinical application of the buccal fat-pad as apedicle graft and to observe healing process. Methods:Cadaver dissection and observation on the boby were performed to sum up the features of the buccal fat-pad as a pedicle graft which was used to repair soft tissue defectsafter the resection of malignant tumors in oral cavity. Clinical application was undertaken in 112 patients with120 flaps. Results:The uncovered buccal fat pad graft underwent eventual epitheliazation by oral mucosa after 6~8 weeks. All cases were followed-up for 3 months to 8 years and 6 months. A good result has been obtained.Conclusion;The advantages of the buccal fat pad graft are that the anatomical region is consistant,and easy toexcise and that the operation can be performed by one incision with good site appearance and minimal functionalloss.
