摘 要:目的为了寻找暴发危险因素,提出控制及预防措施。方法制定病例定义,临床诊断病例为2009年3月2日以来,某厂出现腹泻或腹痛伴头痛、恶心、头晕、发热、呕吐症状之一的人员;确诊病例为临床诊断病例且沙门菌培养阳性的病例。病例搜索通过面对面问卷调查在工厂所有员工中开展。病例对照研究中病例组选取全部病例,对照组选择所有没有发病的病例,通过面对面调查询问危险因素,比较危险因素,用Logistic Regression进行多因素分析。沙门菌用细菌培养检测。结果搜索到病例48例,其中17例为确诊病例。病例对照发现上夜班是危险因素(OR=39,95%CI13~129),上夜班的人群中3月2日的夜宵是危险因素,烧鸭和蒜苗香干是危险食物,非条件Logistic回归多因素分析发现只有蒜苗香干是危险食物。结论这是一起沙门菌污染食物引起的暴发;可能有两次污染即3月1日夜宵和3月2日夜宵。3月2日夜宵的危险食物是蒜苗炒香干。食物污染来源可能是厨师污染引起。Objective To find out the risk factors of salmonella outbreak in a factory in Zhuhai, and to put forward prevention and control measures. Methods Defined probable cases as diarrhea or abdominal pain with one of the followed symptoms: headache, nausea, dizziness, fever, and vomiting since march 2, 2009 in the factory. Confirmed cases as probable cases with salmonella positive. We searched cases in the factory by using case - controlled study. Case group was chosen from all the cases, and the controls were selected from the rest without symptoms, We compared the risk factors between the cases and the controls, analyzed multi - factors by logistic regression analysis method, and tested salmonella by culture. Results Forty- eight cases had been found, including 17 confirmed cases. Case - controlled study showed that night shift was the risk factor (OR:39, 95%CI= 13-129), of the night shift workers, night snack on March 2nd was the risk factor, bean curd slices fried garlic sprout and roast cluck were the risk food. Conclusions This outbreak was caused by salmonella contaminated food, probably contaminated night snack on March 1st and March 2nd. The risk feed on March 2nd was the bean curd slices fried garlic sprout. Food may be contaminated by chef in the factory.
